过敏性紫癜并发急性肠套叠的诊治

Diagnosis and treatment of allergic purpura accompanied with acute intestinal intussusception

目的 探讨过敏性紫癜并发急性肠套叠的临床特点、诊断及外科治疗。方法 对 12例过敏性紫癜并发急性肠套叠患儿的临床资料进行回顾性分析。结果 该病症状不典型 ,小肠套叠多见 ,常发生肠坏死或肠穿孔 ,且发生时间较早。腹部B超检查 9例全部确诊 ;术前漏误诊 3例。全组均行手术治疗 ,无术后并发症和死亡。结论 该病临床症状多不典型。腹部B超检查有确诊价值。

Objective To study the clinical features, diagnosis and surgical therapy of allergic purpura accompanied with acute intussusception(APAI). Methods The clinical data of 12 infants with APAI were analysed retrospectively. Results APAI commonly seen in children and had no typical clinical manifestation. Intussusception and intestinal necrosis or hemorrhage unsually occurred in the early stage of the disease. The diagnosis in 9 cases was confirmed by B type ultrasounography(BUS), and 3 cases misdiagnosed before operation. All patients were cured by operation, no death and no complication occurred. Conclusions The synptoms of this disease is not sepcific; BUS is very useful for the diagnosis. Operation should be done as soon as the diagnosis is confirmed.
