摘要
目的通过调查绝经后妇女日常膳食中各种植物雌激素成分摄入量及骨密度分布情况,分析植物雌激素摄入量与绝经后妇女骨密度的关系,为绝经后骨质疏松的预测及其有效的个体化膳食提供参考依据。方法选取符合调查要求的南昌市160例绝经后妇女,通过食物频率问卷(FFQ)、3 d饮食称重记录、24 h膳食回顾,收集研究对象人口学特征和膳食资料,计算食物中各种植物雌激素的含量。通过双能X线吸收法测量受试者腰椎L2-4和髋部骨密度(BMD)。结果总植物雌激素低、中、高摄入组的腰椎BMD及股骨BMD比较差异均无统计学意义(P>0.05)。异黄酮类植物雌激素低、中、高摄入组的腰椎BMD比较差异有统计学意义(P<0.05),其中L2 BMD比较差异无统计学意义(P>0.05),而L3、L4各组间BMD比较差异均有统计学意义(P<0.05);但3组间股骨BMD比较差异无统计学意义(P>0.05)。香豆素类与木酚素类植物雌激素低、中、高摄入组的腰椎BMD及股骨BMD比较差异均无统计学意义(P>0.05)。结论南昌市绝经后妇女BMD的高低与其膳食中总植物雌激素摄入量的多少无关;但其腰椎BMD的高低与异黄酮类植物雌激素摄入量的多少有关,摄入越多BMD值越高,其中L3、L4最为显著。
Objective To analyze the relationship between phytoestrogen intake and bone mineral density(BMD)in postmenopausal women through investigating dietary phytoestrogen intake and BMD distribution,and to provide a reference for postmenopausal osteoporosis forecast and individualized dietary intervention.Methods According to the requirement,160 postmenopausal women were investi-gated.Phytoestrogen content of foods was determined by food frequency questionnaire(FFQ),3-day wei-ghed diet records,24-hour dietary recall method of collecting diet information,demographic characteristics,and dietary data.The BMD was measured by dual-energy X-ray absorptiometry at the lumbar spine(L2-4)and hip.Results No differences were found in lumbar spine and femoral BMD among different contents(low,medium and high)of total phytoestrogen intake,as well as among different contents of coumarins and lignans(P0.05).There were significant differences in BMD at lumbar spine L3-4,but not L2,among low,medium and high total phytoestrogen-intake groups(P0.05).But there were no differences in femoral BMD among the three groups(P0.05).Conclusion There is no correl-ation between BMD and total phytoestrogen intake in postmenopausal women in Nanchang.However,isoflavone intake is positively correlated with BMD at lumbar spine,especially at L3-4.
