摘要
目的观察IgA肾病(IgA nephropathy,IgAN)患者外周血中Th1、Th2、Th3细胞亚群的表达,探讨其在IgAN免疫发病机制中的作用。方法采用流式细胞仪检测23例IgAN患者(IgAN组)及20例健康体检者(正常对照组)外周血Th1、Th2、Th3细胞比例,并用Spearman或Pearson相关分析法对其分布变化与IgAN各项临床指标进行相关性分析。结果 IgAN组外周血中Th1细胞比例较正常对照组减少,差异无统计学意义(P>0.05);IgAN组外周血中Th2、Th3细胞比例均显著高于正常对照组,差异均有统计学意义(均P<0.05),且与血清IgA水平呈正相关;IgAN组存在Th1/Th2细胞比例失衡,Th1/Th2细胞比值较正常对照组明显下降,差异有统计学意义(P<0.05)。结论 IgAN患者外周血中Th2、Th3细胞比例增多,可能参与IgAN的免疫发病机制。
Objective To observe the levels of Th1,Th2 and Th3 subsets in the peripheral blood in patients with IgA nephropathy(IgAN),and to explore their roles in the immune pathogenesis of IgAN.Methods The percentages of Th1,Th2 and Th3 cells in the peripheral blood were measured by flow cytometry in 23 IgAN patients(IgAN group)and 20 healthy subjects(control group).The correlation between the changes in distribution of Th1,Th2 and Th3 cells and clinical parameters of IgAN was analyzed by Spearman or Pearson rank correlation test.Results The percentage of Th1 cells in IgAN group was lower than that in control group,but there was no significant difference between the two groups(P0.05).Compared with control group,IgAN resulted in a significant increase in the percen-tages of Th2 and Th3 cells but an obvious decrease in the Th1/Th2 ratio(all P0.05).Furthermore,the percentages of Th2 and Th3 cells were positively correlated with serum IgA levels.Moreover,IgAN patients had Th1/Th2 imbalance(P0.05).Conclusion The increased percentages of Th2 and Th3 cells in the peripheral blood may play a role in the pathogenesis of IgAN.
