摘要
目的探讨蒙古族人群血管炎症标志物水平与不同亚型高血压的关系。方法以年龄≥20岁蒙古族居民作为研究对象,采取调查问卷方式收集人口统计学特征和生活方式危险因素资料,测量身高、体质量和血压,检测血脂、血糖、胰岛素和炎症标志物等指标水平。采用单因素方差分析和多因素协方差分析方法研究炎症标志物水平与单纯收缩期高血压(ISH)、单纯舒张期高血压(IDH)和复合型高血压(SDH)的关系。结果经年龄与性别调整后,各亚型组C-反应蛋白(CRP)水平显著高于正常组,ISH组和SDH组的血清可溶性细胞间黏附分子(sICAM)-1水平高于正常组,IDH和SDH组的可溶性E选择素(sE-selectin)水平高于正常组(均P<0.05);经多因素调整后,ISH组[lnCRP1.99(1.86~2.12),P=0.075]和SDH组[lnCRP1.95(1.89~2.02),P<0.05]的CRP水平高于血压正常组[lnCRP1.86(1.83~1.90)],IDH组的sICAM-1水平[313.21(301.16~325.26)μg/L]低于血压正常组[328.65(323.68~333.61)μg/L](P<0.05),IDH组的sE-selectin水平[lnE-selectin3.03(2.98~3.08)]高于血压正常组[lnE-selectin2.96(2.94~2.98)](P<0.05)。结论 CRP与ISH关系更密切,sE-selectin与IDH关系更密切。
Objective To examine the association between inflammatory biomarkers and hyperten- sion subtypes among the Mongolian population in China. Methods Mongolians aged 20 and above were served as subjects in this study. Demographic characteristics and lifestyle risk factors were investigated by the questionnaire, and their body height, weight and blood pressure were measured, and blood lipids, blood glucose, insulin, inflammatory markers etc. were examined for all subjects. Analysis of variance and covarianee were used to examine the relationship between inflammatory biomarkers and hypertension subtypes including isolated systolic hypertension (ISH), isolated diastolic hypertension (IDH) and com- bined systolic and diastolic hypertension (SDH). Results After adjustment for age and gender, C-reac- tive protein(CRP) was higher in all hypertension subtypes than that in normal blood oressure group, intercellular adhesion molecule-1 ( sICAM-1 ) was higher in ISH and SDH group than that in normal blood pressure group, sE-selectin was higher in IDH and SDH group than that in normal blood pressure group. After adjustment for multivariate, CRP was higher in ISH [ lnCRP1.99 ( 1.86 - 2.12 ), P = 0.075 ] and SDH group [ lnCRP1.95 ( 1.89 - 2.02), P 〈 0.05 ] than that in normal blood pressure group [ lnCRP1. 86( 1.83 - 1.90) ]. sICAM-1 level was lower in IDH group [313.21 (301.16 - 325.26) μg/L] than that in normal blood pressure group [ 328.65 (323.68 - 333.61 ) μg/L] (P 〈 0.05). sE-selectin was higher in IDH group [ lnE-selectin3.03 (2.98 - 3.08 ) ] than that in normal blood pressure group[ lnE-se- lectin2.96 (2.94 - 2.98 ) ] (P 〈 0.05 ). Conclusion CRP level was more related to ISH while sE-selec- tin was more related to IDH.
