侧卧位机械通气治疗肺内ARDS临床观察

Clinical observation of the lateral recumbent position ventilation treatment on acute respiratory distress syndrome

目的:分析侧卧位机械通气治疗肺内ARDS的临床疗效。方法:选取于2010年1月至2011年1月在郑州人民医院就诊的患有急性呼吸窘迫综合症的患者30例,使用机械呼吸机对其进行侧卧机械通气治疗,同时使用监控仪采集血氧指数、呼气末肺容积EELV、血流动力学数据,观察不良反应事件。结果:侧卧位通气明显提高了氧合指数和EELV(P<0.05),同时对血流动力参数和肺机械力参数无明显影响(P>0.05),治疗过程中未发现任何不良反应事件。结论:侧卧位通气和呼吸支持相结合可以有效改善ARDS的临床疗效。

Objective： To analyze the effect of lateral recumbent position ventilation on ARDS.Methods： 30 patients with ARDS from Jan.2010 to Jan.2011 in our hospital were treated with lateral recumbent position ventilation using mechanical ventilator.Oxygenation index,end-expiratory lung volume,hemodynamic data were obtained and the adverse events were observed for patients.Results： The oxygenation index and end-expiratory lung volume were improved by the means of lateral recumbent position ventilation（P〈0.05）,and the hemodynamic data as well as the pulmonary mechanical parameters were not influenced（P〈0.05）.No adverse events were occurred in the course of treatment.Conclusion： The combination of the lateral recumbent position ventilation and respiratory support has effectively improve the treatment of ARDS.
