无创呼吸机对冠心病伴重度阻塞性睡眠呼吸暂停综合征患者治疗的血脂成分的影响

The Effect of Noninvasive Ventilator on Serum Lipids in Elderly Patients with Coronary Heart Disease Acompanied with Severe Obstructive Sleep Apnea Syndrome

目的：研究无创呼吸机对冠心病伴重度阻塞性睡眠呼吸暂停综合征（OSAS）患者血清总胆固醇、三酰甘油、低密度脂蛋白、高密度脂蛋白、载脂蛋白A1及载脂蛋白B的影响。方法：选择经多导睡眠图确诊的中重度OSAS患者40例，随机分为治疗组和患病组，每组20名患者，同时选取20名同年龄段，营养状况相似的单纯冠心病患者作为对照组。治疗组采取无创呼吸机进行干预，4周后抽血检测血清血脂成分。结果患病组血清总胆固醇、三酰甘油、低密度脂蛋白均高于对照组和治疗组，高密度脂蛋白低于对照组和治疗组（P〈0．05）；治疗组血清总胆固醇、甘油三酯、低密度脂蛋白高于对照组（P〈0．05），高密度脂蛋白低于对照组，但无统计学意义（P〉0．05）；载脂蛋白A1和载脂蛋白B在各组间均无统计学差异（P〉0．05）。结论：无创呼吸机治疗能够改善冠心病伴阻塞性睡眠呼吸暂停综合征患者血清血脂成分。

OBJECTIVE： To explore the effect of noninvasive ventilator on total cholesterol, triglyceride, low density lipopro- tein, high density lipoproteins, apolipoprotein a and apolipoprotein b in elderly patients with coronary heart disease acompanied with severe obstructive sleep apnea syndrome （OSAS）. METHODS ： 40 cases of OSAS patients confirmed by PSG were selected and divid- ed into the treatment group and disease group randomly, meanwhile, 20 cases with OSAS as control group. After 4 weeks of treatment composition of serum lipids were detected. RESULTS： Compared with disease group, TG, TC, LDL and lipoprotein α were lower in control group and treatment group （P 〈 0. 05）, and HDL was higher （P 〈 0. 05）. Compared with control group, TG, TC, LDL and lipoprotein ct were higher in the treatmentroup （P 〈 0. 05 ） , HDL was lower. There was no significant difference. CONCLUSION ： Noninvasive ventilator can improve the lipid composition in elderly patients coronary with heart disease acompanied with OSAS.
