利用DWI和FLAIR预测急性脑梗死发病时间并指导溶栓治疗的临床观察 被引量:3

摘要 目的探讨利用磁共振弥散加权成像(DWI)和液体衰减反转恢复成像(FLAIR)预测急性脑梗死发病时间并指导溶栓治疗。方法对发病时间已知的患者行MR DWI和FLAIR成像,分别统计分析其阳性结果与发病时间的关系,并寻找一个切点,如果这个切点以内的患者绝大多处于发病6h以内,则对这部分患者进行溶栓治疗。结果 63例患者DWI序列在发病6h以内阳性率为90.00%,6h以后阳性率100%,FLAIR序列在发病6h以内阳性率为9.09%%,6h以后阳性率96.08%。结论在无法获得发病时间时,若DWI为阳性,FLAIR为阴性的患者则大多处于发病6h内(溶栓时间窗内),对这部分患者可进行溶栓治疗。 Objective To explore the Time of onset of acute cerebral infarction by the MR DWI and FLAIR forecast and guidance of thrombolytic therapy. Methods Forward-looking 60 patients with known time of onset of the new admis- sions emergency line DWI-and FLAIR examination, respectively, statistical analysis of its positive results with the onset of the relationship, and find a cut point,if the cutoff point within the vast majority of patients within 6 hours of onest, patients with this part of the thrombolytic. Results 60 patients with DWI sequence-postitive rate of 90.00% within 6 hours,6 hours after the positive rate of 100%, FLAIR sequence-postitive rate ot9.09% within 6 hours, 6 hours after the positive rate of 96.08%. Conclusion Unable to obtain the time of onset, the DWI-positive of FLAIR-negative patients were mostly in the incidence of six hours(within the thrombolytic time window) ,This part of the patients thrombolytic therapy.
