乳腺癌组织中Survivin、Ki67的表达变化及意义 被引量:10

摘要 目的观察乳腺癌组织中凋亡抑制蛋白生存素(Survivin)、细胞核增殖抗原(Ki67)的表达变化,并探讨其意义。方法采用免疫组化法检测10例正常乳腺组织(正常组)、20例乳腺增生性病变组织(增生组)和82例乳腺癌组织(肿瘤组)中的Survivin、Ki67,分析两指标的相关性;观察Survivin阳性表达与乳腺癌临床病理参数及预后的关系。结果与正常组比较,增生组及肿瘤组Survivin OD值、Ki67标记指数(LI)明显升高(P均<0.01);与增生组比较,肿瘤组Survivin OD值、Ki67 LI明显升高(P均<0.01)。Spearman相关性分析显示Survivin表达与Ki67表达呈正相关(r=6.821,P<0.05)。Survivin阳性表达与乳腺癌TNM分期及淋巴结转移相关(P均<0.05)。与Survivin阴性表达患者比较,Survivin阳性患者5年生存率明显降低(P<0.05)。结论乳腺癌组织中Survivin、Ki67表达增加;Survivin和Ki67的高表达在乳腺癌的发生、发展中起重要作用,两者可作为判断其预后的参考指标。 Objective To investigate the expression and significance of apoptosis inhibitor protein Survivin and prolif-erating cell nuclear antigen Ki67 in tissue of breast carcinoma and its clinicopathological significance. Methods The expression of Survivin and Ki67 in 10 cases of normal breast tissues( normal group) , 20 cases of proliferative breast disease (proliferation group)and 82 cases of breast carcinoma tissues (carcinoma group) were detected by immunohistochemical method. Correlation of the two indexes was analyzed, and the relationship between Survivin expression and clinicopathologi- cal parameters and prognosis in breast cancer was observed. Results OD value of Survivin and Ki67 LI in carcinoma group and proliferative group was significantly higher (all P 〈 0.01 ) than that in normal group; Compared with proliferative group, Survivin OD value and Ki67 LI in carcinoma group was significantly higher (all P 〈 0.01 ). Spearman correlation a- nalysis showed that the expression of Survivin and Ki67 was positively correlated (r = 6. 821, P 〈 0.05 ). Survivin expres- sion was related to clinical stage and lymph node metastasis. The 5-year survival rate was significantly higher (all P 〈 0.05) in Survivin negative patients than that in positive patients. Conclusions Expression of Survivin and Ki67 in tissue of breast carcinoma increases; the high expression of Survivin and Ki67 may play an important role in the oncogenesis and development of breast carcinoma and can be used as prognostic predictor of this tumor.
