不同来源肾性蛋白尿与血脂组分及超敏C反应蛋白水平相关性研究

摘要 目的研究分析肾小球性与肾小管性蛋白尿和血脂、超敏C反应蛋白水平之间的相关性。方法对本院肾内科120例肾病患者进行蛋白尿来源的分析,共分为3组:肾小球性62例、肾小管性19例,混合性39例,分别测定24h尿蛋白定量(UTP)、胆固醇(TC)、三酰甘油(TG)以及超敏C反应蛋白(Hs-CRP),另纳入120名正常体检者的4项检测数据作对照。结果经过实验室检测,与对照组相比,120例肾病患者的UTP明显上升(P<0.05);肾小球性组患者的TC、TG以及Hs-CRP明显上升(P<0.05);肾小管性患者的Hs-CRP明显上升(P<0.05);混合组患者的TG、Hs-CRP明显上升(P<0.05)。3组比较,肾小球性患者的TC较其他两组上升明显(P<0.05);TG方面,肾小球性与混血性上升明显(P<0.05);肾小球性患者的Hs-CRP未见明显上升(P>0.05),而其他两组则明显上升。结论肾小球性、混合性患者存在程度不一的脂类代谢异常;Hs-CRP的检查发现,3组患者均有不同程度的炎症状态,但以肾小管性为最。因此,改善血脂和超敏C反应蛋白的水平,能有效缓解尿蛋白,临床改善肾病患者的病情。 Objective To study and analyze the correlation among glomerular and tubular proteinuria and blood lipid and Hs-CRP. Methods One hundred and twenty cases of nephritic patients from nephrology department of our hospi- tal were divided into 3 groups, glomerular group with 62 cases, tubular group with 19 cases, and mixed group with 39 cases. Their proteinuria sources were analyzed, 24 hours UTP, TC, TG and Hs-CRP were detected and compared with four test data of 120 healthy controls. Results Through laboratory detection and compared with control group, UTP of 120 nephritic patients rose significantly (P 〈 0.05). TC, TG and Hs-CRP of glomerular group rose significantly (P 〈 0.05); Hs-CPR of tubular group rose significantly (P 〈 0.05). TG, Hs-CRP of mixed group rose significantly (P 〈 0.05). Among three groups, TC of glomerular group rose significantly compared with the other two groups (P 〈 0.05). TG of tubular and mixed group rose significantly (P 〈 0.05). Hs-CRP of glomerular group didn't rise significantly (P 〉 0.05), but the other two groups were on the contrary. Conclusion Glomerular and mixed group have different levels of disorders of lipid metabolism. Hs-CRP test revealed that three groups all have different levels of inflammatory status but tubular group is the most. So improving the levels of blood lipid and Hs-CRP can effectively relieve proteinuria and improve the patient's condition.
