奥曲肽治疗上消化道出血疗效观察

Efficacy observation of octreotide in treatment of upper gastrointestinal bleeding in 56 cases

目的探讨奥曲肽对急性上消化道出血的临床疗效。方法选取2011年6月～2012年2月在本院治疗的上消化道出血患者56例,随机分为对照组和治疗组各28例,对照组给予常规治疗和普通止血药,治疗组在常规治疗的基础上给予奥曲肽0.1mg+0.9%氯化钠溶液20mL缓慢静脉注射,然后将0.1mg的奥曲肽加入250mL葡萄糖或盐水中维持静脉滴注(>25μg/h),每瓶滴注4h,24h总量为0.6mg,用药2～3d。比较两组的临床疗效。结果治疗组总有效率为96.4%,对照组总有效率为85.7%,两组经统计学分析差异有统计学意义(P<0.05)。结论奥曲肽治疗上消化道出血起效快、方法简单、不良反应少,临床疗效显著,值得临床推广应用。

Objective To study the clinical curative effect of octreotide for acute upper gastrointestinal hemorrhage. Methods Fifty-six patients with upper gastrointestinal bleeding from June 2011 to February 2012 in our hospital were selected for treatment, and were randomly divided into control group and treatment group with 28 cases in each group, control group were given routine treatment and the general hemostatic drugs, the treatment group were treated with rou- tine treatment based on given octreotide O. 1 mg ＋ 0.9%NaC1 20 mL slow intravenous injection, then 0.1 mg octreotide joined 250 mL glucose or saline maintenance intravenous infusion （〉 25 p^g/h）, one bottle infusion of 4 h, 24 h total of 0.6 rag, use 2-3 days. The clinical effects of two groups were compared. Results The total effective rate of treatment group was 96.4%, the total effective rate of control group was 85.7%, two groups had statistically significant difference （P 〈 0.05）. Conclusion Octreotide in the treatment of hemorrhage of upper digestive tract has advantages of fast, sim- ple method, less adverse reactions, significant clinical effect, is worthy of clinical application.
