产科送检的74份不合格标本的原因分析和解决措施

Cause analysis and countermeasures of 74 unqualified obstetric specimens

目的统计深圳西乡人民医院产科送检的不合格检验标本,分析其产生的原因,并制订相应的整改措施,以提高产科送检标本的合格率。方法回顾性分析产科送检的74例不合格检验标本,根据不合格的原因进行分类,并分析不合格标本的分布情况及产生原因,并针对性地采取相应的措施。结果产科送检的74例不合格标本分中,标本量不足(22份)、标本与检验申请单姓名不符(14份)和标本怀疑有污染(10份)居不合格率的前3位;不合格标本送检率最高的是血常规标本和尿常规标本,其次为凝血四项。通过实施相应的整改措施,2011年7～12月产科送检的不合格标本显著减少。结论分析检验标本不合格的原因,进行相对应的护理干预,并加强与检验科的沟通,通过完善标本采集和运送的操作规程,可减少不合格送检标本的发生。

Objective To investigate the unqualified obstetric specimens in Xixiang People＇s Hospital in Shenzhen City, to analyze its causes and to deveIop appropriate corrective measures, thus to improve the pass rate of obstetric specimens. Methods 74 cases of unqualified obstetric specimens were retrospectively analyzed and classified accord- ing to the causes. The distribution and causes of the unqualified specimens were analyzed and targeted appropriate measures were carried out. Results Among the 74 cases of unqualified specimens, insufficient specimens （22）, mis- matches between tags on specimens and test request forms （14） and suspected pollution （10） were top 3 causes of dis- qualification; the highest unqualified rates were of routine blood specimens and urine specimens, followed by four co- agulation test specimens. After the implementation of appropriate corrective measures, the number of unqualified ob- stetric specimens was significantly reduced from July to December in 2011. Conclusion The analysis of the causes for unqualified obstetric specimens, the implementation of correspondent nursing interventions, the improvement of com- munication with the laboratory department and the standardization of specimen collection and transportation are useful for reducing the occurrence of unqualified specimens.
