鹰嘴牵引加小夹板固定治疗儿童肱骨髁上GartlandⅢ型骨折

Treatment of Gartland Ⅲ supracondylar fracture of the humerus in children using traction of o- lecranon combined with small splints fixation

目的探讨鹰嘴牵引加小夹板固定治疗儿童肱骨髁上GartlandⅢ型骨折的临床疗效。方法83例儿童肱骨髁上GartlandⅢ型骨折者均行尺骨鹰嘴牵引治疗5～7d后，在麻醉下手法复位加小夹板外固定3～4周，并分期功能锻炼。结果83例均获随访，时间6～48个月，参照Flynn标准：优62例，良16例，可4例，差1例，优良率为93．9％。无严重肘内翻畸形发生。9例合并神经损伤，伤后2—3个月恢复正常。结论该方法操作简便，安全可靠，疗效确切，并发症少。

Objective To investigate the clinical effect of treatment of Garfland Ⅲ supracondylar fracture of the humerus in children using traction of olecranon combined with small splints fixation. Methods Eighty-three cases of Gartland Ⅲ supracondylar humeral fracture were treated by traction of olecranon for 5 - 7 days. Then they were given anaesthesia and treated by manipulative reduction combined with small splints fixation for 3 -4 weeks. All cases were permitted function exercise by stages. Results All cases were followed up for 6 - 48 months. According to the standard of Flynn, 62 cases obtained excellent results, 16 good, 4 fair and 1 poor. The rate of excellent or good was 93.9%. No serious cubitus varus was found. Nine cases who had partial nerve injury and got back to normal in 2 -3 months after injury were recorded. Conclusions This operation is simple and safe, the fixation is firm, effective and with few complications.
