临床药师参与排除1例疑似他汀相关性药疹的药学实践

The participation of clinical pharmacist in exclusion of suspected statin-related drug eruption,a case report

目的:报道1例临床药师通过用药调查排除的疑似他汀相关药疹病例,探讨临床药师在药疹诊断中的方法与作用。方法:冠心病患者,71岁,男性,在支架植入术后的长期他汀降脂治疗过程中反复出现皮疹。患者既往无过敏性疾病及食物过敏史。医生疑其皮疹与他汀相关而调整降脂方案。临床药师通过对患者既往用药史及过敏史的调查,依据药疹发生的一般规律排除了患者反复皮疹与他汀类药物的相关性。结论:临床药师掌握药疹的鉴别与处理,可弥补非专科医生用药史采集、药疹诊断知识欠缺的不足。

Objective： To investigate the role and the way of clinical pharmacists in the diagnosis of drug eruption through reporting the participation of clinical pharmacist in a suspected case of statin-related drug eruption.Results： In a 71-years-old male patient with coronary heart disease,rash occurred several times in the course of statin lipid-lowering therapy after stent implantation.The patient had no previous history of disease and food allergies.The physicians suspected that the rash was associated with statin,so adjusted lipid-lowering treatment.Based on the general properties of drug eruption and through investigating the medication history of the patient,the clinical pharmacists excluded the relation of rash with statins.Conclusion： The clinical pharmacists who master the identification and treatment of drug eruption can make up for the physicians＇ lack of drug eruption diagnosis and medication history collection.
