双C方案治疗难治性糖尿病的临床体会

摘要 目的总结动态血糖监测系统(CGMS)及胰岛素泵皮下持续输注系统(CSII)组成的双C方案治疗难治性糖尿病患者的临床体会。方法将36例难治性糖尿病患者随机分为双C组20例和每日多次胰岛素皮下注射组(MSII组)16例。比较两组治疗2周后的血糖控制情况、达标时间、胰岛素用量、日间及夜间血糖波动幅度、低血糖发生情况等。结果与MSII组相比,双C治疗组血糖达标时间短,胰岛素用量少,低血糖的发生率低,血糖波动幅度小(P<0.05)。结论双C方案治疗难治性糖尿病效果优于MSII,患者预后好。 Objective To summary the clinical experiences of double C program which be consisted of continuous glucose monitoring system and continuous subcutaneous insulin infusion pump system in blood glucose control of patients with refractory diabetes mellitus. Methods Thirty six patients with refractory diabetes mellitus were randomly divided into double C group with 20 cases and multi subcutaneous insulin injection (MSII) group with 16 cases. The status of controlling blood glucose, time of reaching the expected level, insulin dose, amplitude of glycemic excursions day and night and hypoglycemia were measured and compared after treatment for two weeks. Results Compared to MSII group, the double C group had the character of less time of reaching the expected level, lower insulin dose, lower frequency of hypoglycemic reactions, smaller fluctuations in blood glucose (P 0.05). Conclusion Double C program is superior to MSII in treatment of refractory diabetes mellitus, and it has the better prognosis.
