院前急救对高血压脑出血患者预后的影响探讨

Influence of pre-hospital first-aid on prognosis for patients with hypertension intracerebral hemorrhage

目的探讨院前急救对高血压脑出血患者预后的影响。方法回顾性分析2010年1月~2012年1月我院急诊科收治的210例高血压脑出血患者的临床资料,其中入院前实施过院前急救者为院前急救组（n=110）,未实施院前急救而由家属送入院的为对照组（n=100）。比较两组治疗有效率、并发症发生率及生活质量等。结果院前急救组在发病后到达急诊科时间（t1）、急诊科接诊后进行初步处理时间（t2）、发病后送达科室并得到确切治疗的总时间（t5）及各类并发症发生率均短于对照组（P〈0.05或P〈0.01）,血压控制率、有效率、生活质量各项评分均高于对照组（均P〈0.05）。两组均无死亡病例。结论院前急救能有效降低高血压脑出血患者的死亡率及并发症发生率,提高其抢救有效率及生存质量。

Objective To explore the influence of pre-hospital first-aid on prognosis for patients with hypertension intracerebral hemorrhage.Methods Clinical data of 210 patients with hypertension intracerebral hemorrhage treated in Department of Emergency of our hospital from January 2010 to January 2012 were analyzed retrospectively,110 cases were given the dispose of pre-hospital first-aid（pre-hospital first-aid group）,100 cases were not given the dispose of pre-hospital first-aid and sent to the hospital by family members（control group）.The effective rate,the incidence of complications and the quality of life between the two groups were compared.Results t1（after the onset time to arrival at the emergency department）,t2（after emergency department to initial treatment time）,t5（the total time of service department to get the exact treatment） and all kinds of complications in the first-aid group were shorter those that in control group（P 0.05 or P 0.01）,the rate of blood pressure control,efficient,the scores of life quality were higher than those in the control group（all P 0.05）.Both groups had no deaths.Conclusion Pre-hospital first-aid can effectively reduce the mortality rate and complications incidence and improve the efficiency of rescue and quality of life.
