摘要
目的探讨腹膜透析前内皮素-1(ET-1)和血管内皮生长因子(VEGF)在慢性肾功能衰竭(CRF)患者腹水中表达水平与腹膜转运功能的关系,从而前瞻性预测腹膜功能。方法共收集48例腹膜透析前患者腹水,腹膜透析1个月后行改良腹膜平衡试验(改良PET),记录超滤量。根据腹膜平衡试验结果将患者分为高转运组10例、平均转运组(高平均转运、低平均转运)31例和低转运组7例。另留取同期10例肾病综合征患者腹水为对照组。以ELISA法检查各组标本中ET-1和VEGF的蛋白质水平。结果①VEGF的蛋白质水平的差异在实验组间、实验组与对照组间均有统计学意义(P<0.05)。②ET-1蛋白质水平的差异在实验组间无统计学意义(P>0.05);在实验组与对照组间有统计学意义(P<0.05)。③净超滤量与透析液VEGF和ET-1的浓度均呈负相关。结论 ET-1和VEGF的蛋白质水平结合可用于评价腹膜的转运功能。
Objective To investigate the relationship between endothelin-1(ET-l) and vascular endothelial growth factor(VEGF) expression level and transport function in chronic renal failure(CRF) patient's ascites before peritoneal dialysis,so as to predict the function of peritoneal prospectively.Methods 48 cases of patient's ascites were collected before peritoneal dialysis.After commencing peritoneal dialysis for one month,a modified peritoneal equilibration test was performed,and ultrafiltration volume was recorded.According to the peritoneal equilibration test results,the patients were divided into the high transshipping group 10 examples,the average transshipping group(high average transportation,low average transportation) 31 examples and the low transshipping group 7 examples.In addition,10 cases of nephrotic syndrome patient's ascites were collected as the control group.The levels of proteins of ET-1 and the VEGF in each group were inspected by the ELISA.Results ①There was significant difference with the levels of VEGF protein in the experimental group,and between the experimental group and control group(P 0.05).②There was no significant difference with the level of ET-1 protein in the experimental group(P 0.05);There was significant difference with the level of ET-1 protein between the experimental group and control group(P 0.05).③Net ultrafiltration volume was negatively correlated with the VEGF and ET-1 concentration.Conclusion Combination of ET-1 and VEGF protein levels can use to evaluate the peritoneal transport function.
