机械通气联合纳洛酮治疗急性呼吸衰竭疗效分析

Effect of mechanical ventilation combined with naloxone for patients with acute respiration failure

目的探讨机械通气联合纳洛酮治疗急性呼吸衰竭(ARF)的临床效果。方法 49例ARF患者分为对照组24例和观察组25例,对照组患者在原发病治疗基础上给予糖皮质激素、纠正水和电解质紊乱、维持酸碱平衡等综合治疗,并行面罩机械通气;观察组在对照组治疗的基础上给予纳洛酮治疗;对2组患者治疗效果、血气分析及转归进行比较。结果观察组患者治疗有效率显著高于对照组(P<0.05);2组患者治疗后血酸碱度、动脉血二氧化碳分压、动脉血氧分压和动脉血氧饱和度均显著改善(P<0.05),观察组改善程度更显著(P<0.05);且观察组患者痊愈出院率显著高于对照组(P<0.05),病死率显著低于对照组(P<0.05)。结论机械通气联合纳洛酮治疗ARF疗效显著,可降低患者病死率。

Objective To explore the effect of mechanical ventilation combined with naloxone for acute respiration failure （ARF）. Methods Forty-nine patients with ARF were divided into control group （ n = 24） and observation group （ n = 25 ）. The patients in control group were treated with combined therapy, such as the treatment of protopatby, glueocorticoid, electrolyte disturbances correction, acid-base balance maintenance and mechanical ventilation. Based on these ,tile patients in observation group were treated with naloxone. The therapeutic efficacy, results of blood gas analysis and prognosis of the two groups were compared. Results The effective rate in observation group was significantly higher than that in control group（ P 〈 0. 05 ）. After treatment, The power of hydrogen, partial pressure of carbon dioxide in artery, arterial partial pressure of oxygen and ＇arterial oxygen saturation in the two groups were improved significantly （P 〈 0.05 ）, and the improvement was more in observation group （P 〈 0.05 ）. The healing rate of patients in observation group was significantly higher than that in control group （ P 〈 0.05 ）, and the case fatality was significantly lower in observation group than that in control group（ P 〈 0.05 ）. Conclusion Mechanical ventilation combined with naloxone for the treatment of patients with ARF can obtain significant effect, and degrade case fatality.
