多层螺旋CT在宫内节育器异位诊断的应用价值

The Value of MultiSlice Spiral CT in the Diagnosis of Ectopic Intrauterine Contraceptive Device

目的探讨多层螺旋CT在宫内节育器(IUD)嵌顿、异位及其并发症诊断中的应用价值。方法对2010年1月至2012年4月在本院检查并治疗的26例IUD异位患者的临床及影像资料进行回顾性分析,并与术中所见进行对照。所有病例均行CT及超声检查,11例另行X线摄片。结果 26例中IUD嵌顿于子宫肌层10例,其中4例穿破浆膜层;16例异位于子宫外,其中9例位于盆腔内,4例嵌顿于肠壁,2例嵌顿于膀胱,1例位于右侧腹腔肋弓下。所有病例中异位IUD均手术取出,手术所见与CT诊断符合率为100%。结论多层螺旋CT不仅在IUD异位诊断方面可提供精确、直观、立体的图像,同时可对其并发症做出正确的术前评估。

Objective To evaluate the value of MSCT in the diagnosis of embedded and ectopic intrauterine contraceptive device（IUD）.Methods 26 cases with ectopic IUD during 2010.1~2012.4 were selected,the CT manifestations and intraoperative findings of 26 cases were compared.All cases received MSCT and Ultrasound.11 cases received X-ray examination.Results Of 26 cases,there were 10 cases of ectopic IUD embedded in the womb muscle layer and ectopic IUD was perforated through the uterine serosa in 4 cases;There were 16 cases of ectopic IUD outside the uterus： 9 cases with ectopic IUD in the pelvic cavity,4 cases with ectopic IUD embedded in the intestinal wall,2 cases with ectopic IUD embedded in the bladde,1 case with ectopic IUD displaced around the intestine in the right abdomen and the arcus costarum.All patients received surgical treatment.The diagnosis accordance between intraoperative findings and CT reached 100%.Conclusion MSCT could provide accurate,intuitive,stereo images in diagnosis of ectopic IUD,also make the correct preoperative evaluation of the ectopic IUD complications.
