摘要
目的通过对脑血管容积CT数字减影血管成像(VCTDSA)启动扫描时间影响因素的研究,确定主要影响因素及其间的相关性。方法前瞻性研究脑血管VCTDSA检查病例115例,记录患者的性别、年龄、身高、体重、心率、对比剂注射部位、对比剂浓度与体积、注射速率与持续时间、盐水冲洗量与速率,有无吸烟史、高血压病史、冠心病、糖尿病、高血脂、颅内动脉瘤、颅内动脉狭窄、颅内血管畸形,计算身高体重指数,对上述因素与启动扫描时间的关系进行分析。结果对性别、对比剂注射部位、吸烟史、高血压病史、冠心病等计量资料采用t检验进行组间比较,P值均>0.05。采用多元逐步回归分析定量资料(入选标准为P=0.1,剔除标准P=0.15),身高、对比剂浓度、对比剂体积、注射持续时间、心率、盐水冲洗量P值均<0.1;Beta值分别为0.1176,-0.1486,0.4071,0.3173,-0.2625,0.2306。参考公式:启动扫描时间(s)=9.4142+身高(cm)×0.0424-对比剂浓度(mgI/ml)×0.0328+对比剂体积(ml)×0.2051+注射持续时间(s)×0.3231-心率(次/分)×0.0537+盐水冲洗量(ml)×0.0957。结论身高、对比剂浓度、对比剂体积、注射持续时间、心率、盐水冲洗量是启动扫描时间的主要影响因素,其中,对比剂体积和注射持续时间(即注射速率)对启动扫描时间的影响最大;余因素对启动扫描时间影响不大。
Objective To investigate the influencing factors of time to start scanning in cerebral vessels in VCTDSA,and determine the main factors and the correlation between them.Methods 115 cases of cerebral vascular VCTDSA were selected,patient's gender,age,height and weight,heart rate,venous access site,contrast medium concentration and volume,injection rate and duration time,saline flush volume and rate,smoking history,hypertension,coronary artery disease,diabetes,high blood fat,intracranial aneurysm,intracranial arteriostenosis,intracranial vascular malformations and intracranial tumors were recorded,then the body mass index was calculated.The correlation of the above factors and the time to start scanning were analyzed.Results The qualitative data such as gender,venous access site,smoking history,hypertension,coronary disease and so on were compared between groups with t test,all P0.05.The multivariate gradually regression analysis was adopted for analysing the quantitative data(the entry value P=0.10 and the removal value P=0.15).The P values of height,contrast medium concentration and volume,injection duration time,heart rate,saline flush volume were all0.1.The Beta values of height,contrast medium concentration,contrast medium volume,injection duration time,heart rate,saline flush volume were 0.1176,-0.1486,0.4071,0.3173,-0.2625,0.2306,respectively.Recommended formula of time to start scanning(s)=9.4142+height(cm)×0.0424-contrast medium concentration(mgI/ml)×0.0328+contrast medium volume(ml)×0.2051+injection duration time(s)×0.3231-heart rate(beats/min)×0.0537+saline flush volume(ml)× 0.0957.Conclusion Height,contrast medium concentration and volume,injection duration time,heart rate,saline flush volume are main influencing factors of time to start scanning;and among them,contrast medium volume and injection duration time(also injection rate)are the most important factors of time to start scanning.The other factors have not serious influence.
