摘要
目的探讨皮肌炎患者外周血调节性T细胞/辅助性T细胞17(Thl7)的变化及意义。方法选取36例皮肌炎患者和36名健康对照者,分别采用流式细胞术、实时定量逆转录聚合酶链反应和酶联免疫吸附法法检测外周血调节性T细胞frhl7细胞百分率、核转录因子叉头翼状螺旋转录因子3(Foxp3),视黄醇类核内受体.yt(RORrt)mRNA的表达以及血浆转化生长因子(TGF)-B,、白细胞介素(IL).17的水平。统计学处理采用t检验,相关分析采用Pearson直线相关分析。结果皮肌炎患者CD4+Foxp3+调节性T细胞百分率[(7.2±2.3)%]、Foxp3mRNA表达水平(0.44±0.19)以及血浆TGF-B,水平[(37400±4814)pg/ml]与健康对照组相比显著降低[(9.1±3.2)%、0.62±0.25、(41200~6348)pg/ml,P均〈0.05];与对照组相比,皮肌炎患者外周血中Thl7细胞百分率[(1.22±0.41)%]、ROR,/tmRNA表达水平(0.85~0.34)及血浆IL-17水平[(77~32)p咖1]显著高于对照组[(0.83±0.14)%、0.61i-0.21、(58+20)pg/m~](P均〈0.05)。CD4+Foxp3调节性T细胞百分率、Foxp3mRNA表达水平及血浆TGF.B,水平与血清肌酸激酶水平呈负相关(r==-0.573,-0.593,-0.618,P〈0.05),Thl7细胞百分率、RORr/tmRNA表达水平及血浆IL-17水平与肌酸激酶水平呈正相关(r=0.582,0.609,0.623,P〈0.05)。结论皮肌炎患者外周血中调节性T细胞frhl7细胞失衡可能与皮肌炎的发病有关。
[Abstract] Objective To explore the changes and clinical significance of regulatory T cell/T help cell 17 (Thl7) in the peripheral blood from patients with dermatomyositis (DM). Methods Thirty-six patients with DM were recruited and thirty-six healthy individuals were enrolled as normal controls. The percentage of regulatory T cell/Thl7 cell was assessed by flow cytometry and the mRNA levels of Foxp3/RORrt were detec- ted by real-time quantitative reverse transcription polymerase chain reaction (RT-PCR). The plasma levels of TGF-β1/IL-17 were measured by enzyme linked immunosorbent assay (ELISA). T-test was used for statistical analysis and Pearson's linear analysis were used for correlation analysis. Results The percentage of CD4+ Foxp3+ T cell, the mRNA expression of Foxp3 and the plasma levels of TGF-β1 decreased significantly in pati- ents with DM than in normal controls [ (7.2+2.3)% vs (9.1_+3.2)%, (0.44+0.19) vs (0.62+0.25), (37 400± 4814) pg/ml vs (41 200+6348) pg/ml, P〈O.05]. The percentage of Thl7 cell, the mRNA expression of RORrt and the plasma levels of IL-17 increased in patients with DM compared with normal controls [ ( 1.22± 0.41)% vs (0.83+0.14)%, (0.85+0.34)vs (0.61+0.21), (77±32)pg/ml vs (58+20)pg/ml, P〈0.05]. There was negative correlation between the percentage of regulatory T cell, the mRNA levels of Foxp3, the plasma levels of TGF-β1 and the serum levels of CK in patients with DM (r=--0.573, -0.593, -0.618, P〈0.05). There was positive correlation between the percentage of Thl7 cell, the mRNA levels of RORrt, the plasma levels of IL-17 and the serum levels of CK in patients with DM (r=0.582, 0.609, 0.623, P〈0.05). Conclusion The imbalance of regulatory T cell/Thl7 cell may exist in the peripheral blood of patients with DM. Regulatory T cellIThl7 cell may be associated with the pathology of DM.
