施他宁联合丙氨酰谷氨酰胺治疗危重症感染合并胃肠功能障碍患者的临床疗效观察

Clinical therapeutic effect of Stilamin combined with cammonia alanyl glutamine on patients with severe infection complicated with gastrointestinal dysfunction

[目的]观察施他宁联合丙氨酰谷氨酰胺治疗危重症感染合并胃肠功能障碍患者的临床疗效。[方法]选择208例并发胃肠道功能障碍的危重症感染患者,随机均分为治疗组和对照组,治疗组采用针对原发病的常规治疗和脏器功能的支持治疗,同时给予施他宁和丙氨酰谷氨酰胺;对照组仅给予针对原发病的常规治疗和脏器功能的支持治疗。检测2组患者治疗前后的APACHII评分、丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)、白蛋白(ALB),并观察反流,呕吐、腹痛、腹胀和腹泻等临床症状的改善情况。[结果]治疗组发生反流、呕吐、腹痛、腹泻、腹胀病例明显少于对照组,2组比较差异有统计学意义(P<0.05)。治疗组显效31例,有效56例,总有效率为83.6%,对照组显效17例,有效36例,总有效率为50.9%,2组治疗前后APACHII评分比较均有差异,但治疗组评分差异更明显(P<0.05)。治疗组治疗后ALT和AST显著下降,ALB则明显上升;对照组治疗后ALT、AST、ALB均较治疗前无显著改变。[结论]采用施他宁联合丙氨酰谷氨酰胺的综合治疗措施,危重症感染患者胃肠功能有明显的改善。

[Objective]To observe the effects of Stilamin combined with alanyl glutamine treatment on severe infection complicated with gastrointestinal dysfunction.. I-Methods-J208 patients were randomly divided into treatment group and control group. The treatment group was administered with talanyl gluta-mine and Stilamin, conventional methods for primary disease and organ function support,while the control groupwith conventional methods for primary disease and organ function support. Before and after the treatment, APACHII score,alanine aminotransferase （ ALT ）, aspartate aminotransferase （ AST ）, albumin （ ALB ）, as well as reflux, vomiting, abdominal pain, bloating and diarrhea and other symptoms in both groups were detected.. [Results] The number of patients with reflux, vomiting, abdominal pain, diarrhea, abdominal distension in the treatment group were significantly less than that in the control group（P〈 0. 05）. The total effective rate of the treatment group and the control group was 83.6% and 50. 9% respectively. The APACHII score in the treatment group showed significant difference before and after the treatment（P〈0.05）. After the treatment, the treatment group showed significantly decreased ALT and AST and increased ALB significantly. While those in the control group showed no significant difference.. [Conclusion]Stilamin combined with alanyl glutamine could improve the patients with severe infection compli cated with gastrointestinal function. ]
