摘要
目的了解实施世界银行贷款/英国赠款"结核病防治项目的实施效益。方法根据结核病控制项目的实施方案和世界银行贷款/英国赠款的配套文件要求。在实施中的《监测信息管理系统》的报告运用软件进行分析,利用结核病流行病学调查抽取病案(数据库资料)进行效益评估。结果自2003年正式启动"结核病控制"项目以来,8年共接诊可疑结核病人27865人,被确诊免费治疗(涂阳病人)6675例,通过评价治愈病例5740例,治愈率85.99%。效益评估:接受综合医院住院治疗病例支付6800元/人,项目治疗支付799.66元/人。两者比值为:后者比前者少支付6000.34元/人(占11.76%)的费用。"项目"能为6675例结核病人节约4005.23万元。治愈的5740例结核病人,恢复劳动力后8年可为国家创造直接经济效益5804.80万元。结论我区利用世界银行贷款/英国赠款的《中国结核病控制项目》8年来,使"项目"逐步实现了从专业防治到政府支持,卫生、财政等共同参与的社会防治,使我区结核病疫情得到明显下降,加快了结核病控制工作进程。取得了良好的社会效益和经济效益,达到了"项目"的低投入,高效益的目标。
Objective To understand the effect and profit of tuberculosis control program Loaned by International Bank for Reconstruction and Development/ British grants. Methods According to the requirement of tuberculosis control project plan and the World Bank loan/UK grants supporting documentation, using the software to analyze the report of the monitoring information management system in the implementation, and tuberculosis epidemiology investigation extraction medical record (database) were used to evaluate the effect and profit. Results 27865 patients with suspected tuberculosis are diagnosed with 6675 patients suffering with positive slide and treating free and 5740 patients are cured in the eight years from the project of tuberculosis. Benefit assessment: The patient would pay 6800 RMB who was treated with comprehensive hospital treatment, and when he was treated with the project, the paying is 799.66 RMB. The latter is far better than the former, 6000.34 RMB will be saved. The ratio is 11.76%. 4005.23 million Yuan will be saved for 6675 patients because of the program and the 5740 patients who are cured will create 5804.80 million Yuan in eight years. Conclusion There is an obvious difference in the control of tuberculosis and tuberculosis cases get declined obviously in Hechuan County, and after this program which has brought a lot of benefits.
