河南省AIDS高发区HIV/AIDS患者卫生服务利用分析

Health service utilization of HIV/AIDS in high-incidence area of Henan province

目的:了解河南省AIDS高发区HIV/AIDS患者卫生服务利用情况,为进一步开展AIDS防控和管理工作提供科学依据。方法:选取河南省3个AIDS高发县(上蔡、沈丘和确山),采用多阶段随机整群抽样方法抽取9个乡、27个行政村共900名HIV/AIDS患者作为研究对象进行现场问卷调查。结果:HIV感染者两周就诊率为57.6%,AIDS患者两周就诊率为82.5%,差异有统计学意义(χ2=9.755,P=0.002);AIDS患者在村卫生所和乡卫生院的就诊率高于HIV感染者(P<0.05)。HIV感染者年住院率为19.6%,低于AIDS患者的25.2%(χ2=2.197,P=0.138);二者在乡卫生院、县及以上医疗机构的住院率差异无统计学意义(P>0.05)。结论:HIV/AIDS患者卫生服务利用量较大,不同疾病阶段卫生服务利用不同,不同医疗机构之间卫生服务利用也存在差异。

Aim：To study the health service utilization condition of HIV/AIDS in high-incidence area of Henan,so as to provide certain scientific basis for carrying out AIDS prevention and control as well as management work.Methods： A total of 900 HIV/AIDS were randomly sampled from 27 administrative villages of 3 AIDS counties（ Shangcai,Shenqiu,and Queshan） and a questionnaire survey was conducted.Results： The two-week outpatient rate of HIV and AIDS patients was 57.6% and 82.5% （X2=9.755,P=0.002） ,respectively.The outpatient rate of AIDS patients was higher than that of HIV in rural clinics and township hospitals.The hospitalization rate of HIV and AIDS was 19.6% and 25.2% （X2=2.197,P=0.138） ,respectively.There was no significant difference between HIV and AIDS patients in choosing rural hospitals or county level medical institutions or above for hospitalization.Conclusion： The health service utilization of HIV / AIDS is greater than normal people; different disease stage has different health service utilization,and there is difference in health service utilization among different medical institutions.
