喉罩麻醉与气管插管麻醉在胆囊手术中的效果比较

The Effect Comparison of Laryngeal Mask Anesthesia and Tracheal Intubation in the Gallbladder Operation

目的:探讨喉罩麻醉与气管插管麻醉在胆囊手术中的效果比较。方法:将100例胆囊切除术患者随机分为对照组和观察组,对照组采用气管插管麻醉,观察组采用喉罩麻醉,比较两组患者麻醉情况及血液动力学变化。结果:观察组的拔管时间、清醒时间显著短于对照组,维库溴铵用量显著少于对照组(P<0.05)。两组患者的心率(HR)、收缩压(SPB)、舒张压(DPB)在麻醉诱导前(T0)时无显著性差异(P>0.05)。两组在诱导后(T1)、置喉罩(或插管)后即刻(T2)、置喉罩(或插管)后5 min(T3)、10 min(T4)时刻的SPB、DPB显著低于T0(P<0.05)。观察组在T1、T2、T3和T4的SPB、DPB、HR与对照组的这四个相同时刻点的这三项指标有显著性差异(P<0.05)。结论:喉罩麻醉用于胆囊切除术效果满意,血液动力学较稳定,值得临床推广。

Objective：Objective to investigate the effect comparison of Laryngeal mask anesthesia and tracheal intubation in the gallbladder operation.Methods：100 cases of cholecystectomy patients were divided into the observation group and the control group,the control group general anesthesia by endotracheal intubation,the observation group was treated with laryngeal mask anesthesia,compared two groups of patients with anesthesia,hemodynamic changes.Results：The observation group of extubation time,wake time was significantly shorter than that in the control group was significantly less than that in the control group,vecuronium bromide（P0.05）.In two groups of patients with heart rate（HR）,systolic blood pressure（SPB）,diastolic blood pressure（DPB） before anesthesia induction（T0） when no significant differences （P 0.05）.In two group after induction（T1）,the laryngeal mask（or tube） （T2）,immediately after the laryngeal mask（or tube） after 5min（T3）,10 min（T4） time SPB,DPB was lower than that in T0（P0.05）.The observation group in T1,T2,T3and T4SPB,DPB,HR and control group at the same time points of the three indicators had significant difference（P0.05）.Conclusion：Laryngeal mask anesthesia for cholecystectomy effect is satisfactory,its hemodynamic is stability and it is worthy of clinical application.
