无创呼吸机治疗老年急性Ⅱ型呼吸衰竭患者疗效观察

Non-invasive Ventilation in the Treatment of Elderly Patients with Acute Type Ⅱ Respiratory Failure in Patients with Efficacy Observed

目的:探讨无创呼吸机治疗老年急性Ⅱ型呼吸衰竭患者的临床疗效。方法:40例老年急性Ⅱ型呼吸衰竭患者在常规综合治疗的同时,配合无创呼吸机治疗,观察机械通气前、机械通气后2h、24h及通气结束时的观察临床疗效。结果:与治疗前相比,无创正压通气后2h、24h及治疗结束时,pH值、PaO2明显升高,PaCO2明显下降,差异有统计学意义(P<0.05)。36例患者经治疗后好转出院,4例患者病情逐渐加重,1例因家属不同意气管插管而死亡,另外3例行气管插管机械通气治疗并成功撤机好转出院。结论:无创呼吸机治疗老年急性Ⅱ型呼吸衰竭患者临床疗效肯定,使用方便,值得推广。

Objective：To investigate the clinical efficacy for the treatment of elderly patients with acute type Ⅱ respiratory failure in patients with non-invasive ventilators.Methods：40 cases of elderly patients with acute type Ⅱ patients with respiratory failure in the conventional therapy at the same time,with non-invasive ventilator therapy,the observed mechanical ventilation,mechanical ventilation for 2h,24h and ventilation at the end of clinical effect.Results：Compared with before treatment,non-invasive positive pressure ventilation for 2h,24h and the end of treatment,pH values,PaO2 was significantly increased and PaCO2 decreased,the difference was statistically significant（P0.05）.36 patients improved and discharged after treatment,four cases the patient＇s condition gradually worsened,family members do not agree with endotracheal intubation and death in one cases,the other 3 cases of tracheal intubation with mechanical ventilation in the treatment and the successful weaning improved and discharged.Conclusion：Noninvasive ventilation in the treatment of elderly patients with acute type Ⅱ respiratory failure clinical efficacy is certainly easy to use,it is worth promoting.
