多层螺旋CT血管成像在研究椎动脉先天性发育不良中的应用价值 被引量:2

Application value of multi-slice spiral computed tomography angiography in studying congenital hypoplastic vertebral artery

摘要 【目的】通过对椎基底动脉供血不足患者和健康体检者的椎动脉进行MSCTA定量分析,探讨椎动脉先天性发育不良对发病的影响。【方法】对693例临床诊断或拟诊为椎基底动脉供血不足的患者(病例组)和51例健康体检者(对照组)行头颈部64层螺旋CT血管成像检查。采用超薄层重建,结合三维血管成像多平面重组(MPR)、曲面重组(CPR)和高级血管分析(AVA),得到两组双侧椎动脉正常管腔的直径和横截面积。【结果】病例组693例患者中双侧椎动脉发育正常660例(95.2%);一侧椎动脉发育不良33例(4.8%)。对照组51例(100.0%)健康体检者双侧椎动脉均发育正常。对病例组椎动脉发育正常患者与对照组健康体检者的椎动脉进行直径和横截面积比较,差别无统计学意义(P>0.05)。对病例组椎动脉发育不良患者的发育不良侧椎动脉与对照组健康体检者的相应椎动脉进行直径和横截面积比较,差别有统计学意义(P<0.01)。【结论】64层螺旋CT血管成像在研究椎动脉先天性发育不良对椎基底动脉供血不足患者的病因研究中具有非常重要意义。 [Objective]To evaluate the influence of congenital hypoplastic vertebral artery on vertebrobasilar insufficiency by quantitative analyzing the vertebral artery of vertebrobasilar insufficiency patients and healthy people with 64-slice CTA. [Methods] 693 cases diagnosed or suspected as vertebrobasilar insufficiency patients (case group) and 51 healthy people (control group) were examined by 64-slice CTA. The enhanced CT images of vertebrobasilar artery were postprocessed by use of MPR, CPR and AVA. The normal lumen diameter and cross-sectional area of bilateral vertebral arteries were measured by workstation software and combined with the original axial images. [Results] 660 cases (95.2%) with normal bilateral vertebral artery and 33 cases (4.8%) with one side congenital hypoplastic were observed in 693 patients (case group). Of the 51 cases(control group), 51 persons(100.0%) with normal bilateral vertebral artery. There was no significant difference in the diameter and cross-sectional area of vertebral artery between cases group and control group (P 0.05). However, there were significant differences in the diameter and cross-sectional area of vertebral artery between case group with congenital hypoplastic vertebral artery and control group (P 0.01). [Conclusion] 64-slice CTA has important clinical significance in studying the etiology of congenital hypoplastic vertebral artery in vertebrobasilar insufficiency patients.
