保留残迹前交叉韧带重建对移植物血流量的影响 被引量:7

Effect of preservation of remnant attachment on the blood flow of tendinous graft in anterior cruciate ligament reconstruction

摘要 [目的]探讨保留残迹重建前交叉韧带对移植物血流量恢复的影响。[方法]48只新西兰兔一期行双侧前交叉韧带重建术,一侧保留韧带残端纤维,对侧切除韧带残端纤维。重建术后6、12、18、24周时,采用SPE-CT、激光多普勒血流计定量检测移植物血流量恢复情况。[结果]重建术后6周时SPE-CT检测结果显示保留残迹组移植物血流量较切除残迹组明显增加(P<0.05)。重建术后6、12周时激光多普勒检测发现保留残迹组移植物血流量明显高于对照组(P<0.05)。[结论]保留残迹重建前交叉韧带可以促进移植物血流量恢复。 [ Objective] To study the effect of preservation of remnant attachment on the blood flow of tendinous graft after anterior cruciate ligament reconstruction in a rabbit model. [ Method ] Forty-eight New Zealand rabbits were involved and both knees of rabbits received transection of anterior cruciate ligament simultaneously. One knee was reconstructed with the preserva- tion of residual attachment of ligament, and the contralateral one was treated with the debridement of remnant attachment. The 6, 12, 18, and 24 weeks after reconstruction, the blood flow of tendinous graft was observed in vivo by SPE-CT and laser doppler flow meter respectively. [ Result] The blood flow of grafts in remnant-saved group by SPE-CT testing was significantly increased the 6 weeks after reconstruction as compared with that of remnant-debrided group ( P 〈 0.05 ). Furthermore, the 6 and 12 weeks after reconstruction the blood flows of grafts were found higher in the remnant-saved group than that of control by laser doppler flow meter (P 〈0.05 ). [ Conclusion] The preservation of remnant attachment in anterior cruciate ligament reconstruc- tion could facilitate the recovery of blood supply of tendinous graft.
