胸腔镜在纵膈肿瘤切除术中的临床应用价值

Clinical value of thoracoscope in mediastinal tumor resection

目的探讨胸腔镜在纵膈肿瘤切除术中的临床应用价值。方法 82例纵膈肿瘤患者为观察组,均应用胸腔镜辅助小切口行纵膈肿瘤切除术。另将应用传统开胸手术的40例纵膈肿瘤患者作为对照组。比较2组患者术中、术后情况以及并发症发生率。结果观察组患者术中出血量、手术时间、切口长度、切口疼痛评分、胸腔引流管放置时间、住院时间均显著小于对照组,观察组并发症发生率亦显著低于对照组。结论胸腔镜辅助小切口手术可减少手术创伤,减轻患者痛苦,加速术后恢复,且术后并发症较少,在纵膈肿瘤患者的治疗中具有较高的应用价值。

Objective To explore the clinical value of thoracoscope in mediastinal tumor re- section. Methods Eighty-two patients with mediastinal tumor in our hospital from Jan 2009 to June 2012 were divided into observation group, and all the patients received mediastinal tumor re- section with thoracoscope-assisted small incision. Forty patients with mediastinal tumor before Jan 2009 who received traditional thoracotomy were divided into control group. The perioperative and postoperative situation and adverse reactions of the two groups were compared. Results The bleed- ing amount, operation time, length of incision, incision pain scoring, placement time of drainage tube and length of stay in the observation group were obviously smaller than that in the control group（P 〈 0.01 ）. Conclusion Thoracoscope-assisted small incision can reduce the operation in- jury, relieve the patient pain, accelerate postoperative recovery and has few adverse reactions. It has high application value in the treatment of patients with mediastinal tumor.
