综合康复治疗的介入时间对脑出血患者的影响

Study on effect of intervention time of comprehensive rehabilitation on patients with cerebral hemorrhage

目的分析综合康复介入时间对脑出血患者的预后的影响,探讨脑出血患者康复治疗的时机及方法。方法对2005年1月至2011年6月收治的高血压性脑出血患者278例的临床资料进行回顾性分析,根据综合康复治疗介入的时间分为两组,将入院后2周内开始康复治疗的患者定为早期组,将入院2周以后开始康复治疗的定为常规组。结果两组患者出院时的病死率、植物生存率和重残率差异无统计学意义(P>0.05),早期组中残及良好率优于常规组,差异有统计学意义(P<0.01);两组患者6个月后日常生活能力Ⅳ～Ⅴ级,差异无统计学意义(P>0.05),早期组ADLⅢ～Ⅰ级优于常规组,差异有统计学意义(P<0.05)。结论综合康复治疗的早期介入有助于脑出血患者的康复,在患者病情稳定后应尽早地进行综合康复治疗,以提高患者的整体治愈率。

Objective To analyze the influence of intervention time of comprehensive rehabilitation on patients with cerebral hemorrhage,and to explore the intervention time of rehabilitation and treatment of patients with cerebral hemorrhage.Methods The clinical data of 278 patients with hypertensive cerebral hemorrhage treated in this hospital during January 2005 to June 2011 were retrospectively analyzed.They were divided into 2 groups according to the intervention time of rehabilitation： early group（within first 2 weeks of admission） and control group（2 weeks after hospitalization）.Results There was no statistical difference among mortality rate,vegetative status rate and severe disability rate（P0.05）.The rates of good recovery and moderate disability in early group were better than those of control group and their difference was statistically significant（P0.01）.The number of patients having grades IV-V on activity of daily living between 2 groups had no statistical significance（P0.05）.But the number of patients having grades I-III in early group was higher than that of control group,and their difference had statistical significance（P0.05）.Conclusion The early comprehensive rehabilitation is helpful to the recovery of patients with cerebral hemorrhage.The comprehensive rehabilitation should be early started when patient is in stable condition in order to raise the recovery rate.
