中药内服加穴位注射对慢性荨麻疹患者T细胞亚群的影响

Effect of Chinese Medicine Combined with Acupoint Injection on T cell Subsets in 33 Cases of Chronic Urticaria

目的探讨中药内服加穴位注射治疗对慢性荨麻疹的临床疗效及作用机制。方法将90例荨麻疹患者随机分为中药组、穴位注射组、联合治疗组3组。中药组采用中药汤剂口服治疗;穴位注射组采用苯海拉明注射剂1ml与维丁胶性钙2ml混合,双侧足三里穴及曲池穴位交替注射;联合治疗组中药联合穴位注射治疗,用法同前。3组均14天为1个疗程,每月进行1个疗程治疗,连续3个疗程。另设健康人对照组30例。各组于治疗前后检测外周血T淋巴细胞亚群并观察临床疗效。结果中药组总有效率为57.14%,穴位注射组总有效率为55.17%,联合治疗组总有效率为72.73%,联合治疗组疗效优于中药组和穴位注射组(P<0.05)。治疗前3组患者与对照组比较,CD3+、CD4+、CD4+/CD8+降低;而CD8+增高(P<0.05)。3组治疗后CD3+与治疗前比较差异无统计学意义(P>0.05);3组治疗后CD4+、CD8+、CD4+/CD8+与本组治疗前比较差异均有统计学意义(P<0.05)。3组治疗后CD4+、CD8+、CD4+/CD8+组间比较,差异均有统计学意义(P<0.05)。结论中药联合穴位注射治疗慢性荨麻疹疗效显著,能调节细胞免疫功能,改善患者T淋巴细胞亚群。

Objective To observe the therapeutic effect and mechanism of Chinese medicine combined with acupoint injection in chronic urticaria patients. Methods Ninety cases of urticaria were randomized into group A, group B and group C. Group A was given Chinese medication. Group B was injected with 1 ml diphenhydramine and 2 ml Vitamin D2 and calcium colloidal in jection on Zusanli （ST36, double） and Quchi （Llll, double） alternately. Group C was given Chinese medication and acupoint injection. One treatment course lasted for 14 days. Each group was given one treatment course per month for 3 courses. Thirty healthy controls were selected. The peripheral blood T cell subsets were detected and the therapeutic effect was observed. Re- suits There were 6 cases （21.43%） cured, 10 cases （35.71%） excellent, 5 cases （17.86%） valid and 7 cases （25%） invalid in group A, with a total effective rate of 57.14~. There were 8 cases （27.59%） cured, 8 cases （27.59%） excellent, 5 cases （17.24%） valid and 8 cases （27. 59%） invalid in group B, with a total effective rate of 55. 17%. There were 11 cases （33.33~） cured, 13 cases （39.39%） excellent, 6 cases （18.18%） valid and 3 cases （9.09~） invalid in group C, with a total effective rate of 72.73%. The total effective rate of group C was superior to that of group A and group B （P^0.05）. Compa- ring with the control group, CD3＋ , CD4＋ and CD4＋/CD8＋ was significantly lower in group A, group B and group C （P% 0.05）, but CD8＋ was significantly higher before treatment （P%0. 05）. There was no significant difference between before and after treatment in CD3＋ in group A, group B and group C （P~〉0.05）. There was a significant difference between before and after treatment in CD4＋ , CD8＋ and CD4＋/CD8＋ in group A, group B and group C （P%0.05）. Comparing with group C, there was a significant difference in CD4＋ , CD8＋ and CD4＋/CD8＋ in group A and group B after treatment （P%0.05）. Con- clusion The combo therapy of Chinese medicine and acupoint injection is effective for chronic urticaria. It can regulate cellular immune function and improve T cell subsets.
