转移抑制基因、基质金属蛋白酶-2在食管鳞状细胞癌中表达的意义

Expression of nm23-H1 gene product and MMP-2 in esophageal squamous cell carcinoma and their clinical significance

摘要 目的探讨转移抑制基因(nm23-H1)、基质金属蛋白酶乏(MMP-2)蛋白在食管鳞状细胞癌中的表达及其临床意义。方法采用免疫组化S—P法检测nm23-H1、MMP-2蛋白在58例食管鳞状细胞癌组织和23例正常食管黏膜中的表达。结果nm23-H1蛋白在食管鳞状细胞癌中的阳性率为43.1%,在正常食管黏膜组织中的阳性率为100%,二者比较差异有统计学意义(P〈0.05)。MMP-2蛋白在食管鳞状细胞癌中的阳性率为91.4%,在正常食管黏膜组织中的阳性率为34.8%,二者比较差异有统计学意义(P〈0.05)。nm23-H1、MMP-2在食管鳞癌中的表达与患者性别、年龄及肿瘤大小无关(P〉0.05),与肿瘤的分化程度、浸润深度及有无淋巴结转移有关(P〈0.05),nm23-H1的表达与切端癌残留有关(P〈0.05)。nm23-H1与MMP-2的表达呈负相关。结论nm23-H1、MMP-2均在食管癌的发生发展过程中发挥作用,均可促进远处转移的发生;nm23-H1的表达缺失可能与切端癌残留有关;nm23-H1、MMP-2可作为判断食管癌转移及预后的指标。 Objective To investigate the expression of nm23-H1 and MMP-2 in esophageal squamous cell carcinoma(ESCC) and their clinical significance. Methods Immunochemical S-P method were used to detect the expression of nm23-H1, MMP-2 protein in 58 patients with ESCC and 23 people with normal esophageal mucosa. Results Twenty-five cases (43.1% ) were positive for nm23-H1 staining and 33 cases (56.9%) were negative. The rate of nm23-H1-positive patients was significantly less than that in normal groups whose rate was 100% (P 〈 0. 05). Fifty-three cases (91.4%) were positive for MMP-2 staining and 5 cases (8.6%) were negative, while eight(34. 8% ) of 23 patients were positive for MMP-2 staining and 15 cases (8.6%) were negative. There was significant difference inMMP-2 positive and normal gro analysis of these of nm23-H1 and -proportion of ESCC patients ups(P 〈 0.05). A further data shown that the expression MMP-2 was related to tumordifferentiation degree, invasion depth and lymph node metastasis (P 〈 0. 05 ), and was not related to patients' gender, age and tumor size (P 〉 0.05). The expression of nm23-H1 was related to residual cancer ( P 〈 0.05), and was negatively correlated to expression of MMP-2. Conclusions nm23-H1, MMP-2 play a role in the development and progression of esophageal cancer, which can increase risk of distant metastasis. Residual cancer may be associated with the absent of expression of nm23-H1. The expression of nm23-H1 and MMP-2 protein can be a marker for judging esophageal cancer metastasis and prognosis.
