摘要
目的应用组织多普勒成像技术,探讨不同血脂水平对无冠心病证据人群左心室心肌功能的影响。方法根据不同血脂水平分为3组。组1:低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)<3.37 mmol/L(n=30);组2:LDL-C 3.37~4.12 mmol/L(n=30);组3:LDL-C≥4.14 mmol/L(n=30)。所有患者均接受传统二维彩色多普勒超声心动图、脉冲波组织多普勒成像及背向散射积分检查。结果与组1和组2比较,组3 E/A、Ea/Aa比值明显降低(P<0.01),背向散射积分参数(循环周期变量指数)明显降低(P<0.05)。与组1比较,组2 E/A、Ea/Aa比值降低(P<0.05),而背向散射积分参数无明显差别。结论不同LDL-C水平对左心室心肌收缩和舒张功能有不同的影响,随着LDL-C增高,左心室心肌功能损害程度逐渐加重。
Objective To explore the impact of serum lipid level on left ventricular myocardial function of patients without coronary heart disease via tissue Doppler imaging.Methods Based on different low-density lipoprotein cholesterol(LDL-C),the subjects were divided into three groups,i.e.,group 1 with LDL-C value 3.37mmol/L,group 2 with LDL-C value 3.37-4.12 mmol/L and Group 3 with LDL-C value ≥4.14 mmol/L.All subjects accepted with examination of conventional 2D color-Doppler echocardiography(2D-CDE),pulsed wave tissue Doppler imaging(PWTDI) for the analysis of diastolic function,and integrated backscatter(IBS) for the evaluation of intrinsic contractility and tissue characterization.Results E/A ratio and Ea/Aa ratio,and the cyclic variation index in integrated backscatter were significantly lower(P〈0.01 or 0.05) in group 3 than those in groups 1 and 2.Compared with Group 1,E/A ratio and Ea/Aa ratio were lower(P〈0.05) in group 2,and the cyclic variation index in integrated backscatter was not significantly different(P〉0.05).Conclusion Different LDL-C contents have different impact on the left ventricular myocardial systolic and diastolic functions.Along with the increase of LDL-C content,the injury degree of left ventricular myocardial function is aggravated.
