有限切开复位内固定治疗复杂Pilon骨折疗效分析

Clinical Efficacy of Limited Open Reduction and Internal Fixation for Complex Pilon Fractures

目的:探讨不同手术方法治疗Pilon骨折临床疗效。方法:以我院105例手术治疗的Pilon骨折患者为研究对象,64例行有限切开解剖型钢板或锁定钢板内固定术者为观察组,41例行有限内固定和外固定支架相结合术者为对照组,观察两组临床疗效。结果:观察组优47例(73.4%),总有效率96.9%;对照组优19例(46.3%),总有效率85.4%;两组Mazur评分及总有效率比较差异有统计学意义(P<0.05或0.01)。观察组发生并发症9例(14.1%),骨折平均愈合时间为(18.2±4.3)周,和对照组比较差异均有统计学意义(P<0.05)。结论:有限切开复位内固定治疗Pilon骨折具有创伤小、显露充分、固定可靠、康复快的优点,是一种治疗Pilon骨折的良好手术方法,值得推广。

Objective： To explore the clinical efficacy of different surgical methods for the treatment of Pilon fracture. Methods： Made 105 cases of the Piion fracture patients in the hospital as the research objects, 64 cases as the observation group were treated with limited open reduction and internal fixation, 41 cases as control group were treated with limited internal fixation and external fixator combined surgery, then the clinical efficacy was observed. Results： In the observation group, 47 cases （73.4%） were excellent with 96. 9 % total effective rate;white in the control group, 19 ca- ses （46. 3%） were excellent with 85.4% total effective. The Mazur score and total efficiency difference between the two groups were statistically significant （P〈0. 05 or 0. 01）. In observation group, complications occurred in 9 cases （14. 1 %）, the average fracture healing time was （18. 2 ± 4. 3） weeks, and compared to the control group, the differences were statistically significant （P〈0. 05）. Conclusion： The method of limited open reduction and internal fixation for PiIon fractures had the advantages of little trauma, fully revealed fractures, easily fixed and reliable, and quick recovery, which was a good surgical methods for treatment of Pilon fractures, and worthy of promotion.
