目的纠正不合理静脉用药医嘱,实施干预对策,避免或减少可能发生的不良反应。方法归类、分析、总结及干预不合理静脉用药医嘱。结果溶媒载量过多、过少,选择溶媒载体品种不当的现象多于超剂量使用,配伍不当,用药频次及给药顺序不当的现象。结论药师通过静脉用药集中配置平台及合理用药干预对策实施后,提升了药物安全、有效的治疗水平。

Objective Correct unreasonable intravenous medication orders,implementation of intervention strategies,to avoid or reduce the possible adverse reaction.Method Classification,analysis,summary and the intervention of unreasonable intravenous medication orders.Result The solvent contained too much,too little,choice of solvent carrier varieties improper phenomenon than overdose,incompatibility,medication frequency and delivery sequence improper phenomenon.Conclusion Pharmacists by intravenous medication centralized configuration platform and rational use of drug intervention strategy implementation,enhance the drug safety,the level of effective treatment.
