右美托咪啶对下肢手术硬膜外麻醉效果及止血带疼痛的影响 被引量:1

摘要 目的观察右美托咪啶对下肢手术硬膜外麻醉及止血带疼痛的影响方法择期行胫腓骨手术患者40例,随机双盲分为右美托咪啶组(A组)和对照组(B组),均采用连续硬膜外麻醉。置管后A组静脉注射右美托咪啶0.5μg/kg(生理盐水稀释至10mL),B组静脉注射生理盐水10mL。两组均于置管后30min上止血带开始手术。记录两组患者止血带使用时间及手术时间、出现止血带疼痛的时间、感觉阻滞持续时间和运动阻滞持续时间。分别记录止血带疼痛出现时(T1),止血带充气后1h(T2),术后30min(T3),术后60min(T4)的VAS评分。记录心动过缓、低血压、呼吸抑制的发生情况。结果 A组麻醉维持时间较B组延长(P<0.01);A组止血带疼痛出现时间晚于B组(P<0.01);A组各时点VAS评分均低于B组(P<0.01);两组均未出现心动过缓、低血压、呼吸抑制等不良反应。结论术前使用右美托咪啶可增强下肢手术硬膜外麻醉的效果,减轻止血带疼痛,无明显不良反应。
