我院静脉药物配置中心退药情况及相关差错分析

Analysis of Returned Medicine and Related Medication Errors in Pharmacy Intravenous Admixture Services of Our Hospital

目的分析静脉药物配置中心退药情况及相关用药差错,促进合理用药。方法对2011年9月10日至10月26日内中心退药申请单的数量、代表性退药品种、退药影响因素等进行回顾性分析。重点分析因用药差错导致的退药。结果退药数量占领药数量的9.48%,退药品种以抗生素、中药注射剂为主,退药原因主要有"停药/换药""患者出院""患者死亡"等。因用药差错导致的退药占退药比例的5.03%。结论建议完善各项制度,增强医、护、患之间的沟通,减少用药差错,以此降低甚至避免病房退药的发生。

Objective To analyze the situation of returned medicines and that resulted from medication errors, and to promote rational use of drugs. Methods The number of returned medicine application, types and influencing factors of returned medicine in PIVAS from Sept. 10th to Oct. 26th in 2011 were analyzed retrospectively. Returned medicines resulted from medication error was analyzed according to ASHP medication error classification. Results The number of returned medicines accounted for 9.48% of the number of the total medicines. Main types of returned medicine included antibiotics, TCMI. Main reasons of returned medicine were drug with drawal or change,patient discharge and patient death,etc. Returned medicines resulted from medication error accounted for 5.03%. Conclusion It is suggested to perfect various regulations and strengthen the communication among doctors,nurses and patients to reduce and aviod the occurrence of drugs returned to pharmacy.
