摘要
目的探讨多排螺旋CT靶征诊断急性阑尾炎的价值。方法回顾分析经手术和临床随访证实的58例阑尾炎和24例非阑尾炎患者的MDCT影像表现,探讨阑尾炎靶征的MDCT表现,以及与阑尾炎病理改变的关系。结果阑尾急性炎症所致阑尾浆膜层、黏膜层充血,黏膜下层水肿,MDCT静脉增强表现为靶征。本组患者靶征阳性14例(急性单纯性阑尾炎患者2例,急性蜂窝织炎性阑尾炎8例,急性坏疽性阑尾炎2例,慢性阑尾炎1例,非阑尾炎患者1例),诊断阑尾炎的符合率、敏感度和特异度分别为43.9%、22.4%和95.8%,多见于急性阑尾炎(85.7%)。结论阑尾炎MDCT靶征特异性高,有助于急性阑尾炎的诊断。
Objective To investigate the diagnostic value of the target sign in acute appendicitis with multidetector CT. Methods MDCT of 58 patients with confirmed appendicitis and 24 patients without appendicitis was reviewed. Results The target sign on contrast-enhanced MDCT was related to inflammatory congestion of serosa and mucosa as well as submueosal edema. This sign was more common in patients with acute appendicitis （85.7%） including acute simple appendicitis （2）, acute phlegmonous appendicitis （8）, acute gangrenous appendicitis （2） than chronic appendicitis （1） and non-appendicitis （ 1 ）. The target sign had diagnostic accuracy of 43.9%, sensitivity of 22.4%, and specificity of 95.8%. Conclusion The target sign is helpful for diagnosing acute appendicitis.
