益气活血方预防髋部骨折术后下肢深静脉血栓形成的临床研究

Yiqi Huoxue Fang for preventing lower extremity deep vein thrombosis after surgery of hip fracture

目的观察中药益气活血方预防髋部骨折术后下肢深静脉血栓形成(DVT)的临床疗效和应用价值。方法将146例患者随机分为2组,其中中药组73例口服益气活血方,对照组73例皮下注射低分子肝素钙,连续用药3周后,化验D-dimer、行双下肢血管彩色多普勒超声检查,比较治疗前后D-dimer变化,观察术后是否有出血倾向,并统计DVT发生情况。结果中药组、对照组用药前后D-dimer均显著下降(P<0.05);中药组未发生不良事件,对照组出现1例轻度脑出血、1例广泛皮下出血。结论 2组均有效地降低了DVT的发生率,益气活血方预防DVT是安全的、有效的。

Objective To observe the preventive effect of Yiqi Huoxue Fang on lower extremity deep vein thrombosis （DVT） after surgery of hip fracture, and discuss the clinical application values. Methods The patients （n = 146） were randomly divided into two groups, and Chinese medicinal group （n = 73 ） was orally given Yiqi Huoxue Fang and control group （ n = 73 ） was given injection of low molecular heparin calcium. After continue medication for 3 weeks, the patients were given D-dimer test and double lower extremity vascular color Doppler uhrasonography. The changes of D-dimer values were compared before and after treatment, bleeding tendency was observed after the surgery and occurrence of DVT was counted. Results The values of D-direct were all decreased significantly in two groups （P 〈 0.05 ） be- fore and after medication. There were no adverse events in Chinese medicinal group, and there was one case of mild cerebral hemorrhage and one case of extensive subcutaneous hemorrhage in control group. Conclusion The occurrence rate of DVT effectively decreases in two groups, so Yiqi Huoxue Fang is safe and effective in the prevention of DVT.
