外伤性脾破裂180例诊治体会

目的:探讨外伤性脾破裂诊断和治疗措施及治疗效果。方法:收治外伤性脾破裂患者180例,保守治疗40例,行脾修补术25例,脾部分切除术20例。全脾切除加自体脾移植95例。结果:本组患者均痊愈出院,无1例术后再出血,住院7～17天,输血最多1400ml。术后48～72小时均拔除腹腔引流管,无发热及切口感染,出院前常规B超检查均无腹腔积液及脾周脓肿。随访6～12个月无1例出现并发症。结论:对于外伤性脾破裂要根据患者不同情况和治疗原则采取保守治疗或手术治疗。

Objective：To explore the diagnosis and treatment of traumatic rupture of treatment effect.Methods：Our hospital from January 2000 to August 2012,180 patients admitted traumatic rupture of conservative treatment of 40 cases,20 cases of the row spleen repair 25 cases,partial splenectomy.Full splenectomy plus the splenic autotransplantation 95 cases.Results：The patients were cured,and no case of postoperative bleeding,hospitalization for 7 to 17 days,blood transfusion up to 1400ml.48～72h after removal of peritoneal drainage tube,no fever,and wound infection,prior to discharge conventional B-mode ultrasound was no ascites and splenic abscess.Were followed up for 6 to 12 months,no case of complications.Conclusion：For traumatic rupture of different situations and principles of treatment based on the patient to take a conservative or surgical treatment.
