摘要
目的探讨球帽附着体和磁性附着体覆盖义齿在临床修复中的效果。方法对30例单颌仅存2颗下颌尖牙(无松动)的KennedyⅠ型患者进行磁性附着体和球帽式附着体覆盖义齿(各15例)修复,并于2年后随访,测量义齿的固位力、咀嚼效率,并进行基牙疼痛、基牙松动度、附着体脱落及义齿折裂等指标的临床观察。结果球帽式附着体使用患者中2例出现基牙疼痛,3例出现基牙松动;初始固位力平均(9.032±0.002)N,使用1年后下降为(7.924±0.007)N,2年后下降为(6.988±0.006)N,与初始固位力差异均有统计学意义(P<0.05);咀嚼效率为(67.893±0.015)%。磁性附着体使用患者中无基牙疼痛和基牙松动,初始固位力平均为(7.311±0.005)N,使用1年后下降为(6.512±0.006)N,2年后下降为(6.022±0.003)N,与初始固位力差异均有统计学意义(P<0.05);咀嚼效率为(66.278±0.008)%,二者均无附着体脱落和义齿折断。结论磁性附着体和球帽式附着体覆盖义齿咀嚼效率相当,差异无统计学意义(P>0.05),相较于球帽式附着体,磁性附着体覆盖义齿对基牙侧向力更小,能有效地保护基牙,固位力相对稳定、持久。
Objectives To investigate the clinical application effect of the ball attachment and the magnetic attachment overdenture in the clinical prosthetics.Methods 30 patients with only 2 mandibular canines in single jaw without loosing(Kennedy I) were repaired with magnetic attachment(15 cases)and ball(15 cases)to cover denture respectively and measured the retention force of dentures and the masticatory efficiency ater 2-year follow up.The indicators including abutment teeth pain,degree of abutment teeth loose,attachment falling off and denture jackknifing were observed.Results In the patients with the ball attachmen,2 cases appeared abutment tooth pain and 3 cases appeared the butment tooth loose.The average initial retention force was(9.032±0.002)N,which reduced to(7.924±0.007)N after 1 year and(6.988±0.006)N after 2 years with statistical significance in difference(P0.05) and the chewing efficiency was(67.893±0.015)%.In the patients with magnetic attachment,no butment pain and no abutment tooth loose appeared,the average initial retention force was(7.311±0.005),which reduced to(6.512±0.006)N after 1 year and(6.022±0.003)N after 2 years with statistical significance in difference(P0.05);the masticatory efficiency was(66.278±0.008)%.No attachment falling off and denture broken appeared in both groups.Conclusion The masticatory efficiency of the magnetic attachment and the ball attachment in covering denture is similar and the difference has no statistical significance.Compared with the ball attachment,the magnetic attachment overdenture has less lateral force on abutment teeth,and can effectively protect abutment teeth with relatively stable and persistent retention force.
