探讨尿路感染细菌尿与脓尿的关系

摘要 目的:探讨尿路感染细菌尿与脓尿的关系。方法:195例患者于治疗前认真清洗和消毒会阴部,然后取中段尿送培养,有细菌生长者作菌落计数及药物敏感试验,依据培养结果将患者分为无菌尿(尿中无细菌生长)、低菌尿(尿中细菌<105个/ml)及高菌尿(尿中细菌≥105个/ml)3组。将全部患者的脓尿按以下标准分为3组;轻度脓尿为尿液中白细胞5~20个/HP;中度为21~30个/HP;重度为>30个/HP。将脓尿与细菌数量的差异作显著性检验。结果:195例中,轻度脓尿55例,中度60例,重度80例。无菌尿109例,低菌尿48例,高菌尿38例。结论:对尿路感染患者的脓尿及菌尿均应认真对待,严重再发性感染应精密随访,加强治疗及预防。反复发作者应当用抗生素半年以上。应鼓励患者大量饮水并经常排空膀胱。性交后排空膀胱可减少再感染机会,但睡前服1次预防量的抗菌药可能会更有效。大多数尿路感染患者的尿培养细菌计数低于105/ml,因此,关于细菌尿对有症状患者的诊断价值,值临床医师和微生物学家进一步研究。 Objective:To investigate the bacteria in urinary tract infection of urinary and pyuria relationship.Methods:195 cases of patients before treatment and careful cleaning and disinfecting perineum,and then remove the urine to culture,there was bacterial growth for colony counting and drug sensitive test,based on culture results were divided into sterile urine(urine no bacterial growth),low bacteriuria(urinary bacterial105/ml) and high bacterial urine(urine bacterial≥105/ml) 3 groups.All patients with pyuria according to the following standards are divided into 3 groups;mild pyuria as urine white blood cell 5~20/HP;moderate for 21-30/HP;severe 30/HP.The pyuria and bacterial number difference significance test.Results:in 195 cases,mild pyuria in 55 cases,moderate in 60 cases,80 cases of severe.Sterile urine in 109 cases,48 cases of low urinary tract,high urinary tract in 38 cases.Conclusion:the patients with urinary tract infection of pyuria and bacteriuria should be treated seriously,severe recurrent infections should be precise follow-up,strengthen the prevention and treatment of.Authors should be repeated with antibiotics for more than half a year.Patients should be encouraged to drink lots of water and regular emptying of the bladder.Fucked after emptying the bladder can reduce the chance of infection,but before bedtime a prevention of antimicrobial drugs may be more effective.Most urinary tract infections in patients with urinary bacteria count of less than 105/ml,so on bacterial urine on patients with symptoms diagnosis value,value to clinicians and microbiologists to further study.
