前列腺增生电切术后二次电切20例分析

ANALYSIS ON THE 20 CASES ACCEPTED THE SECOND TRANSURETHRAL RESECTION OF PROSTATE FOLLOWING THE FIRST TURP

目的探讨良性前列腺增生(BPH)经尿道前列腺电切(TURP)术后再次出现排尿困难并需要第二次电切的发生原因及处理方法。方法回顾性分析20例良性前列腺增生TURP术后排尿困难行第二次电切术的病例资料。结果根据不同的病因采取不同的手术方式及后续治疗,痊愈17例,耻骨上膀胱造瘘2例,1例死亡。结论正确的术前诊断,依据不同的病因选择对应的手术方式、加强术后处理,必要的术后随访,是第二次手术成功并提高老年人生活质量的关键,同时也是进一步减少第二次手术机会的关键。

Objective To discuss causes and treatment for the dysuria that needed a second transurethral re- section of prostate（TURP） following the first TURP on benign prostate hyperplasia （BPH）. Methods 20 cases who have recurent dysuria received second TURP, the clinical data was retrospectively analysised. Results According to the different condition, appropriate treatment was admitted to these patients. In a follow -up of about 24 months, 17 cases cured, suprapubic cystostomy in 2 cases, 1 patient died. Conclusion Correct preoperative diagnosis, proper treatment according to the different condition, rigorous postoperative regulation and follow up, was the key step to en- hance the quality of life for these patients, and decrease the necessary for second TURP.
