前列腺液中性粒细胞弹性蛋白酶在Ⅲ型前列腺炎诊断中的意义

The significance of neutrophil elastase in the diagnosis of type Ⅲ prostatitis

目的:探讨中性粒细胞弹性蛋白酶(neutrophil elastase,NE)在Ⅲ型前列腺炎诊断中的意义。方法:ⅢA型36例与ⅢB型87例分别进行前列腺按摩液(expressed prostatic secretion,EPS)细菌培养、EPS常规检查、NIH-前列腺炎症状评分(NIH-CPSINE)、NE浓度的测定,然后行独立样本t检验和简单相关分析。正常对照组84例同样进行上述检查后分别与ⅢA型和ⅢB型进行独立样本t检验。结果:①ⅢA型和ⅢB型患者EPS中NE(t=6.32,P<0.05)差异有统计学意义,NE与白细胞呈现明显的正相关关系(r=0.878,P<0.05),两者CPSI(t=1.85,P>0.05)比较差异无统计学意义。②ⅢA型和对照组患者EPS中白细胞(t=13.74,P<0.05)、NE(t=6.499,P<0.05)、CPSI(t=23.76,P<0.05)差异有统计学意义。ⅢB型和对照组患者EPS中NE(t=2.14,P<0.05)、CPSI(t=33.15,P<0.05)差异有统计学意义,白细胞(t=1.49,P>0.05)无统计学意义。结论:NE在Ⅲ型前列腺炎的诊断中能够作为一个很有意义的指标,而NE浓度与白细胞呈较强的正比关系,提示NE浓度的高低能够反映出慢性前列腺炎的严重程度。

Objective：To explore the significance of Neutrophil Elastase （NE） in the diagnosis of type Ⅲ pros tatitis. Method：The level of NE, EPS routine, bacterial culture and CPSI were first detected in 36 cases of type Ⅲ A and 87 cases of type Ⅲ B, then the results of two groups were analyzed by independent samples t-test and the relation between level of NE and the number of leukocyte was evaluated. Normal control group of 84 eases were done by same method of two experimental groups, and the results were analyzed by independent samples t-test witb the experimental group respectively. Result： ①There were significant differences in level of NE （ t = 6.32, P 〈0.05） between Ⅲ A and Ⅲ B groups . There was a obviously positive relation between the level of NE and the number of leukocyte in test group （ r=0.878, P〈0.05）. TheCPSI （t=1.85, P〉0.05） between Ⅲ Aand Ⅲ B groups was not statistically significant. ②The level of leukocyte （ t = 13.74, P 〈0.05）, NE （ t = 6.499, P 0.05）, CPSI （ t =23.76, P 〈0.05） were statistically significant between Ⅲ A and the control group. The NE concentration （ t=2. 14, P 〈0.05） and CPSI （ t=33. 15, P 〈.0.05） were statistically significant between Ⅲ B and control group, while the number of leukocyte （ t =1.49, P〉0.05） was not statistically significant. Conclu-sion：The data presented here indicated that the level of NE in EPS has some significance in the diagnosis of type Ⅲ A and ⅢB prostatitis. There was a strong positive relation between NE concentration and the number of leuko- cyte, which showed the level of NE concentration could reflect tile severity of chronic prostatitis.
