ATF7在结直肠癌临床预后评价中的价值

摘要 目的探讨活化转录调控因子(ATF)/CREB家族一员ATF7在结直肠癌预后中的价值。方法收集72例随访完整且有病理切片的患者,用免疫组化的方法检测其ATF7的表达情况。分析ATF7的表达与患者临床病理学因素的相关性及其对总生存(OS)和无进展生存(PFS)的影响。结果 72例结直肠癌患者中,有43例(59.7%)表达ATF7,有29例(40.3%)未表达ATF7,其表达和临床病理分期有关(P=0.041);表达ATF7的患者对比未表达的患者五年生存率分别为74.4%、41.4%(P<0.002),两者的5年无进展生存率分别为69.8%和37.9%(P<0.002)。单因素分析显示影响PFS和OS的因素包括:ATF7的表达,淋巴结受侵情况(有无淋巴结受侵、受侵数目),病理学分期和癌周淋巴结,多因素分析显示:ATF7的表达和淋巴结受侵数目影响患者OS(P=0.010、P=0.001),ATF7的表达和有无淋巴结受侵影响患者PFS(P=0.007、P<0.001)。结论结直肠癌组织中ATF7与临床病理学分期呈负相关,与OS和PFS呈正相关,ATF7和淋巴结受侵情况共同影响患者的生存率。
