摘要
目的探讨空腹血糖受损(IFG)患者臂踝动脉脉搏波传导速度(baPWV)的变化。方法随机分层抽取唐山开滦(集团)有限责任公司在职及离退休职工中年龄≥40岁、既往无缺血性脑卒中、短暂性脑缺血发作、心肌梗死者共5440例为调查对象,排除资料不完整者,最终纳入统计分析的为5192人。进行血液生化及baPWV检查,根据2003年美国糖尿病学会IFG诊断标准分为理想血糖组、IFG组、糖尿病组,用多因素Logistic回归分析IFG与baPWV的关系。结果与理想血糖组比较,IFG组baPWV较高[左侧:(1580±379)比(1516±380)cm/s;右侧:(1568±344)比(1507±368)cm/s;均P<0.05]。校正了年龄、性别、吸烟、饮酒、收缩压、体质量指数、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、C反应蛋白后,IFG仍是baPWV增加的独立危险因素(OR=1.314,95%CI1.104~1.563)。结论 IFG是baPWV增加的独立危险因素。
Objective To explore the changes of brachial-ankle pulse wave velocity(baPWV) in patients with im- paired fasting glucose (IFG). Methods A total of 5440 subjects who were aged over 40 without previous stroke, transient ischemic attack (TIA), and myocardial infarction were randomly stratified selected from the serving and retired workers of Tangshan Kailuan Company. Those with incomplete record were excluded, and 5192 subjects were recruited in the study. The subjects received blood biochemical and baPWV examination. Then based on the 2003 American Diabetes Association diagnostic criteria for IFG, they were divided into ideal fasting glucose group, IFG group and diabetes group. The relationship between IFG and baPWV was analyzed using Logistic regression. Results The baPWV in IFG group was higher than that in ideal fasting glucose group [left side: (1580±379) vs (1516±380) cm/s, right side: (1568±344) vs (1507±368) cm/s, all P〈0.05]. After adjusting for age, gen- der, smoking habbit, alcohol consumption, systolic blood pressure, body mass index, total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol and C reactive protein, IFG was still an inde- pendent risk factor for the increase of the baPWV value(OR= 1. 314, 95% CI 1. 104-1. 563). Conclusion IFG is an independent risk factor for the increase of baPWV.
