血浆脂蛋白（a）与缺血性卒中新TOAST分型的相关性

Association of Plasma Lipoprotein（a） Levels with the New TOAST Classification in the Patients with Acute Ischemic Stroke

目的探讨血浆脂蛋白（a）[Lp（a）]与缺血性卒中新TOAST各分型的相关性。方法170例急性缺血性卒中患者依据新TOAST标准进行分型，检测各亚型Lp（a）水平，分析Lp（a）与各亚型及病情严重程度的相关性。结果该研究急性缺血性卒中的构成中，最多为动脉粥样硬化性血栓形成（atherothrombosis，AT）64例、其次为小血管病变（smallarterydisease，SAD）46例、其他原因明确的卒中（strokeofotherdeterminedetiology，SOD）29例、心源性栓塞（cardioembolism，CE）26例、和原因不明性卒中（strokeofundeterminedetiology，SUD）5例。与对照组相比，急性缺血性卒中Lp（a）水平升高有统计学意义，各亚型中，AT组Lp（a）水平最高，与其它各亚型相比有统计学差异。以AT为研究对象，Lp（a）水平与病情严重程度为正相关关系。结论血浆Lp（a）的水平与新TOAST分型中AT亚型及病情严重程度密切相关，为及时准确的病因学分析提供一定的指导意义。

Objective To investigate the association of plasma Lipoprotein（a）（Lp（a）） levels with the new TOAST classification in the patients with acute ischemic stroke.Methods 170 acute ischemic stroke patients were classified according to the new TOAST criteria.The levels of plasma Lp（a） in the patients were detected,and the association between the levels of Lp（a）,subtypes and the severity of the disease was analyzed.Results According to the TOAST criteria,atherothrombosis（AT） were 64,small artery disease（SAD） were 46,the stroke of other determined etiology（SOD） were 29,cardioembolism（CE） were 26,and the stroke of undetermined etiology（SUD） were 5,among 170 patients.The levels of plasma Lp（a） were significantly different between the patients and the health controls.The levels of plasma Lp（a） in patients with AT were the highest.There were significant difference between AT and the other subtypes.The levels of Lp（a） were the positive association with the severity of the disease in the patients with AT.Conclusion The plasma Lp（a） levels of patients with acute ischemic stroke were closely associative between AT and the severity of the disease.And it may provide some guides for timely and accurate etiological typing.
