摘要
目的评价和比较血清IGF-1及CEA、CYFRA21。1联合检测、氟.18-氟代脱氧葡萄糖18F-FDG)PET/CT及IGF.1联合18F.FDGPET/CT~种方法早期诊断肺癌的临床价值。方法对确诊的18例肺癌患者和1例肺良性病变患者行PET-CT扫描并行血清学IGF.1、CEA和CYFRA21.检测,比较IGF.1+CEA+CYFRA21.1、PET/CT、IGF。1+PET/CT三种方法对肺癌的检出率。结果IGF-1+cEA+cYFRA2l-1组合对肺癌诊断的灵敏度、准确率和假阴性率分别为83.3%(15/18)、78.9%(15/19)和16.7%(3/18);18F—FDGPET/CT显像分别为88.9%(16/18)、89.5%(17/19)和11.1%(2/18);IGF.1+18F.FDGPET/CT组合分别为88.9%(16/18)、89.5%(17/19)和11.1%(2/18)。结论IGF-1+CEA+CYFRA21.1可作为一种很好的筛查肺癌方法;PET/CT显像对肺癌的诊断有较高的价值;IGF-1与PET/CT联合不能提高诊断肺癌的效能。
Objective To evaluate the diagnostic value of IGF-1 combined with serum tumor markers CEA and CYFRA21-l,positron emission tomography/computed tomography with fluorine-18-labeled fluoredeoxyglucose(lSF-FDG PET/CT),and IGF-1 combined with 18F-FDG PET/CT on lung cancer.Method 18F-FDG PET/CT scan was performed in 18 lung cancer patients diagnosed and 1 patients with benign pulmonary disease.The serum levels of IGF-1,CEA and CYFRA21-1 were detected in the same persons.The sensitivity, accuracy and false positive rate of the combination of IGF- 1,CEA with CYFRA21-1,PET/CT and the combination of IGF-1 with PET/CT were analyzed.Results The sensitivity, accuracy and false positive rate of the combination of IGF-1 with serum tumor markers CEA and CYFRA21-1 was 83.3%(15/18),78.9%(15/19) and 16.7%(3/18),respectively;and those of PET-CT scan was 88.9%(16/18),89.5%(17/19) and 11.1%(2/18),respectively;those of the combination of IGF- 1 with PET/CT scan was 88.9%(16/18),89.5 %(17/19) and 11.1%(2~18),respectively.Conclusion The combination of IGF-1 with serum tumor markers CEA and CYFRA21-1 was an effective screening method on lung cancer.PET/CT was of great value for the diagnosis of lung cancer;Combination of IGF-1 with PET/CT scan cannot enhance significantly diagnostic efficiency on lung cancer.
