CT联合常规模拟机定位在食管癌精确放疗中临床价值研究

Clinical Value of CT Positioning Combined with the Conventional Simulator in the Accurate Radiotherapy of Esophageal Cancer

目的探讨CT定位联合常规模拟机定位在食道癌精确放疗中的临床应用价值。方法回顾分析2009年1月～2012年2月收治的167例局部晚期食管癌的患者资料，全部患者均行真空垫固定体位，并在相同同体位下进行CT定位和常规模拟机定位，通过比较两种定位方式的优缺点，探讨两者联合在食管癌精确放疗中的临床价值。结果与模拟机定位相比，CT定位在显示肿瘤向外侵犯、纵隔淋巴结累及、照射剂量分布等方面有优势；而模拟机定位在明确食管癌的侵犯长度及食管气管或纵膈瘘等方面较CT定位有优势。结论CT联合模拟机定位在食道癌精确放疗中有临床应用价值。

Objective To investigate the clinical value of CT positioning combined with the simulator in accurate radiotherapy of esophageal cancer.Methods Between Jan. 2009 and Feb. 2012,167 patients with local advanced esophageal cancer received both CT and simulator localization at the same body position fixed by vacuum pad.Through the comparative advantages and disadvantages of the two positioning methods,we explored the clinical value between CT and simulator positioning in accurate radiotherapy of esophageal cancer. Results Compared with simulator, CT positioning had advantages such as in the display with the tumor luminal invasion, the mediastinal lymph node involvement and the radiation dose distribution.At the same time,simulator positioning had advantages over CT positioning in the lesion length and the esophageal tracheal or mediastinal fistula.Conclusion CT positioning combined with the simulator is useful in accurate radiotherapy of esophageal cancer.
