摘要
目的探讨DOG1(discovered on GIST 1)蛋白在胃肠道间质瘤(gastrointestinal stro-mal tumors,GIST)组织中的表达情况及临床应用价值。方法应用免疫组化EnVision法检测65例GIST中DOG1和CD117的表达情况并分析DOG1的表达与不同临床病理参数之间的关系,同时检测DOG1在20例胃肠道其它间叶源性肿瘤中的表达情况。结果65例GIST中DOG1和CD117的阳性表达率分别为95.38%(62/65)和93.85%(61/65),两者阳性表达率差异无统计学意义(P>0.05);4例CD117阴性GIST病例均阳性表达DOG1,3例DOG1阴性GIST病例均阳性表达CD117。DOG1在GIST病例不同性别、肿瘤原发部位、组织学分型及Fletcher危险度分级之间比较差异均无统计学意义(P>0.05)。在胃肠道其它间叶源性肿瘤中,DOG1除在1例平滑肌瘤弱阳性表达外,其余7例平滑肌瘤、5例平滑肌肉瘤及7例神经鞘瘤均为阴性表达。结论DOG1是GIST的一个敏感而特异的标记物,联合CD117运用于GIST诊断可以起着互补的作用,有利于提高GIST诊断的准确性,具有较好的临床病理诊断应用价值。
Objective To investigate the expression of DOGl(discovered on GIST-1)in GIST and evaluate its value of diagnostic application. Methods Immunohistochemical EnVision method was used to detect the expression of DOG1 and CDl17 in 65 eases of GIST and 20 cases of other gastrointestinal mesenchymal tumor(GIMT). Simultaneously,to analyze the significance of relationship between the expression of DOG1 and clinicopathologic parameters. Results The positive rates of DOG1 and CDl17 were 95.38 ~ (62/65)and 93.85 ~ (61/65)in GIST respectively, and there were no statistically signifi- cance difference(P〈0.05)of scoring immunohistochemistry between the two markers. High expres- sion(100%, 4/4)of DOG1 was detected in CD117-negtive GIST. Similarly, CDl17 was high expres- sion (100~, 3/3)in DOGl-negtive cases. A significant correlation was not found in GIST between the expression of DOG1 and different sexes, tumor primary sites, categories of cell type and Fletcher's scheme (P〉0.05), respectively. Additionally, DOG1 was weakly stained in 1 of 8 eases of leiomyo- ma, while not stained in 5 cases of leiomyosarcoma and 7 eases of Schwannoma. Meanwhile, CDl17 was focally positive in one case of leiomyosarcoma and was negative in other GIMT. Conclusion DOG1 is a sensitive and specific marker for GIST. With their complementary role, the combination of DOG1 and CDl17 antibody will help to confirm precise diagnosis of GIST.
