头孢哌酮/舒巴坦治疗脑卒中合并肺部感染的临床分析

Clinical analysis of cefoperazone-sulbactam in treatment of pulmonary infections in patients with cerebral stroke

目的比较头孢他啶和头孢哌酮/舒巴坦治疗脑卒中合并肺部感染患者的临床疗效,为控制医院感染提供依据。方法将167例脑卒中合并肺部感染的患者分为对照组和观察组,对照组83例给予头孢他啶静脉滴注治疗,观察组84例给予头孢哌酮/舒巴坦治疗,比较两组患者痰培养细菌清除率和治疗总有效率。结果两组患者治疗前经痰培养共分离病原菌167株,其中头孢他啶组细菌清除率66.3%,头孢哌酮/舒巴坦组细菌清除率达88.1%,两组细菌消除率比较差异有统计学意义(P<0.05);头孢他啶组患者痊愈率为32.5%,总有效率为75.9%,头孢哌酮/舒巴坦组患者痊愈率为47.6%,总有效率为94.0%,两组总有效率比较差异有统计学意义(P<0.05)。结论头孢哌酮/舒巴坦治疗脑卒中合并肺部感染患者疗效好,可作为治疗脑卒中合并肺部感染的经验用药。

OBJECTIVE To compare the clinical efficacies of ceftazidime and cefoperazone-sulbactam in treatment of pulmonary infections in the patients with cerebral stroke so as to provide scientific basis for the control of nosocomial infections.METHODS A total of 167 cerebral stroke patients with pulmonary infections were equally divided into the control group and the observation group,83 cases in the control group were treated with ceftazidime,and 84 cases in the observation group were treated with cefoperazone-sulbactam,the bacterial clearance rate of sputum culture and the total effective rate of treatment between the two groups was compared.RESULTS A total of 167 strains of pathogens were isolated from the sputum specimens before the treatment,the bacteria clearance rate was 66.3% in the control group and 88.1% in the observation group,the difference between the two groups was statistically significant（P0.05）.The cure rate was 32.5% in the control group with the total effective rate of 75.9% and 47.6% in the observation group with the total effective rate of 94.0%,the difference in the total effective rate between the two groups was statistically significant（P0.05）.CONCLUSION Cefoperazone/sulbactam can achieve good clinical efficacy in the treatment of pulmonary infections in the patients with cerebral stoke,which can be used as the empirical medication for the treatment of pulmonary infections emerging with cerebral stoke.
